Associate Practitioner

We are keen to recruit a Band 4 Associate Practitioner to provide specialist input to the Care Home Pathway and the Post Diagnostic Clinic within the North East Hampshire & Farnham CMHTOP.
You will work with professionals from psychology, occupational therapy, nursing and associate nurse practitioners to offer specialist advice and consultation to dementia patients in care homes. You will assist those displaying challenging behaviours using a Positive Behaviour Support framework, support post‑diagnostic work and run/facilitate groups for patients with functional and/or organic presentations.
Main duties of the job
Supportive environment with specialist clinicians, regular training, CPD and supervision. Opportunities to attend quarterly CPD events and share learning.
Travel expected across North East Hampshire and Farnham region, based at Aldershot Centre for Health, Aldershot.
As this role is based in a community setting, you must be able to travel between sites within working hours and may be required to deliver personal care for people with dementia; experience in care settings is advantageous.
Person Specification
Experience
* Demonstrable experience delivering care in a mental health setting.
* Valid driving licence; ability to drive and use a car or motorcycle for work at all times, including community-based work.
Qualifications
* Foundation Degree in Mental Health.
* Related Level 3 qualification in a health-related area and demonstrable experience in a care role.

Post requires a DBS check under the Rehabilitation of Offenders Act (Exceptions Order) 1975.
