We are looking to recruit a Deputy Manager - The Reablement and Hospital Discharge Team within Milton Keynes County Council.
Responsibilities include:
* Working with Vulnerable Adults within a hospital setting
* Report writing and making assessments
* Developing care plans
* Care management
* Managing complex cases
* Liaising with other professional bodies
* Arranging safe and timely discharges for service users
* Self-directed support / personal budgets
* Managing staff
* Provide reflective supervision and support to social workers and social work assistants
* Manage and prioritize allocations based on risk, need, and urgency.
* Support workers in producing strengths-based assessments that accurately reflect the individual and their situation.
* Collaborate with health colleagues and other professionals to ensure timely discharge of individuals.
* Oversee the flow through all discharge services.
* Manage and support a team, addressing HR issues as necessary.
* Support colleagues within a multidisciplinary service.
* Solve problems and manage complex cases and risk.
Experience preferred:
* Post qualifying experience of working as a Senior Social Worker - Hospital Discharge team within the UK Social Care industry.

Minimum Requirements:
* SWE Registration
* 5 year post qualifying experience within UK Social Care industry
* Enhanced DBS/CRB (we can obtain for you)
* Eligibility to work in the UK
