Job Summary
The Groves Hinchley Wood is seeking a motivated Practice Nurse to join our team and provide high-quality care to patients at our Hinchley Wood Surgery.
We are looking for a qualified General Practice Nurse, and nurses with relevant experience who are keen to develop their skills in primary care are encouraged to apply.
This is a part time role, 3 days per week – we are open to discussing flexible working arrangements for the right candidate.

Main duties of the job
* Conduct comprehensive assessments of patients' health needs
* Develop, implement and review individualised care plans
* Provide appropriate treatments and interventions as required
* Undertake treatment room responsibilities and deliver chronic disease management clinics
* Carry out specific clinical tasks including but not limited to:
o Travel health consultations and vaccinations
o Cervical screening
o Childhood immunisations
o Wound management
o Blood pressure monitoring and ECGs
o Venepuncture and health checks
* Work collaboratively with GPs, other nurses and the wider healthcare team to ensure high quality patient care
* Maintain accurate, timely and confidential patient records in line with NHS standards
* Deliver health education advice and counselling to patients and their families
* Keep up to date with current clinical guidelines, medical advancements and best practice in primary care

Person Specification
Essential
* Registered Nurse with current NMC registration
* Evidence of up-to-date NMC revalidation
* Experience in primary care GP setting
* Competence in adult and childhood immunisations and vaccinations
* Cervical cytology training
* Venepuncture skills
* Experience in wound care and dressings
* Ability to manage Chronic Disease Management clinics such as diabetes, asthma and COPD
* Good IT skills and confidence using electronic patient record systems
Desirable
* Travel health training and vaccination experience
* Additional qualifications in chronic disease management such as diabetes or respiratory training
* Evidence of ongoing professional development including CPD courses or certificates
* Mentor or student supervision and assessment qualification such as SSSA
* Ear microsuction certification
