Advanced Nurse Practitioner (Primary Care)
Main responsibilities:
* Lead and manage the Enhanced Health in Care Homes programme, including weekly ward rounds and supporting holistic, person-centred assessments of all patients admitted or readmitted to a care home within 7 days.
* Ensure all residents have a personalised support care plan within 7 days of re/admission.
* Develop and manage highly specialised programmes of care and provide expert advice beyond normal clinical practice scope, including establishing differential diagnoses, management/treatment plans, and making referrals.
* Make fast-track referrals and work autonomously, being accountable for professional actions.
* Undertake advanced clinical nursing practice, assessing patients with acute, non-acute, and chronic conditions, conducting physical examinations, and formulating management plans.
* Manage undifferentiated conditions, identify red flags and serious pathology, and take appropriate actions.
* Use complex decision-making to inform diagnoses, investigations, and management within a broad scope of practice.
* Adopt a personalised and population-centred care approach, supporting shared decision-making with patients.
* Refer patients to other healthcare professionals when appropriate and prescribe medications as needed.
* Maintain accurate records in Emis Web and make critical judgements, including managing acute presentations and making decisions independently when protocols do not exist.
* Visit patients at home and within care homes, communicating complex medical information effectively to patients and carers.
* Promote health, disease prevention, and self-management, recognizing and acting on detrimental health situations.
* Ensure care is evidence-based, aligned with policies, and maintain professional competency for NMC revalidation.
Education and Training:
* Complete relevant training for multi-professional clinical practice and CPD, linking with the Training Hub to promote local training opportunities.
* Organise at least one annual training session for PCN and practice staff on frail elderly care.
Research and Development:
* Engage actively in research, evaluation, and development activities within Connexus PCN, including audits and dissemination of findings.
Leadership:
* Build strong relationships with care home staff to promote good clinical practice.
* Collaborate with GP leads to improve care for elderly residents and provide clinical support to colleagues.
