The successful candidate will assist in the delivery of chronic disease management. General Practice Nurse additional duties wouldalso include: cytology, family planning, child and adult immunisations andwound management. Training in these areas can be given if needed. Experience of the SystmOne clinical system is desirable but notessential as training can be given.

Job responsibilities
JOB PURPOSE:To lead and develop high quality patient care to meet theholistic health care needs of a practice population.To work within NMC Code ofConduct and to agreed practice standards and protocols.
PRIMARY DUTIES AND AREAS OF RESPONSIBILITIES: Key role clinical nursingpractice -The Practice Nurse works independently using own initiative and isaccountable for their own professional actions. Provides support and leadershipfor other members of the Primary Health Care Team and leads towards the co-ordinationof team activities.
Administers adult and child immunisations and vaccinations in accordancewith national and local programmes. Is competent in anaphylaxis andresuscitation techniques. Ensures safe storage, rotation and disposal ofvaccines and drugs and adequate stock levels are maintained.
Offers comprehensive travel health advice for patients prior to travelincluding malaria prophylaxis, safe sex, food hygiene, sun protection, firstaid and emergency medication, health insurance and vaccinations.
Undertakes procedures to include, venepuncture, BP management,urinalysis, peak flow, spirometry, new patient health checks, pregnancytesting, stitch/clip removal, ear syringing and cytology and wound management.
Provides a comprehensive family planning service.
Recognises and understands the importance of observing, documenting andcommunicating the signs and symptoms of child and adult abuse.
Works within all relevant practice and ICB policies and proceduralguidelines e.g. infection control, chaperoning and patient group directions.
Key role care and programme management
Takes a lead in areas such as diabetes, asthma, and COPD management,undertakes the establishment and running of disease management clinics andprovides specialist nursing care for the patients or communities that thepractice serves.
Initiates the process of diagnosis with patients suspected to have achronic disease e.g. diabetes, COPD, asthma, CHD referring to other cliniciansas appropriate.
Manages programmes of care for patients with chronic diseases byplanning, providing and evaluating care under agreed local/national guidelines,protocols and patient group directions.
Takes lead and manages programmes of care for all areas of womens healthincluding providing and evaluating care under agreed local/national guidelines,protocols and patient group directions.
Maintains disease registers.
Audits outcomes of care against standards and initiates changes asnecessary.
Undertakes annual review of patients understanding and ability to selfmanage.
Refers to other members of the extended primary health care team e.g.chiropodist, dietician, specialist nurses.
Key role - Specimens
If needed, to assist with preparation of specimens for collection by thecourier.Deal with any queries arising.
Key role - health promotion -Initiates and carries out programmes ofhealth screening.Contributes to profiling of the practice population.Enables,encourages and supports individuals, families and groups to address issueswhich affect their health and social well-being.Represents individuals andfamilies interests when they are not able to do so themselves.Provides healthylifestyle and smoking cessation advice to individuals and groups.Develops andmaintains the health promotion material within the practice.Makes referrals tostatutory and voluntary agencies as appropriate.
Key role - Infection Control lead -Support patients and colleagues inadopting sound infection control measures. Apply infection control measureswithin the practice according to local and national guidelines. Ensure waste isdisposed of in accordance with practice Infection control policy, and monitorother practice users for compliance with policy. Participate in infectioncontrol audit cycle when required.
Key role manage/lead self/others -
The Practice Nurse has a role in promoting good leadership for allgrades of staff.Contributes to and initiates the development, monitoring andevaluation of care delivery by the Nursing Team.Promotes effectivecommunication and relationships within the team.Works collaboratively as a teammember to achieve the objectives of the GMS contract and aspirations of theteam.Provides expert clinical advice on educational opportunities that willfacilitate the development and support of their specialist knowledge and skillsto ensure the development of clinical practice. Partakes in clinical supervisionfor own development. Contributes to the appraisals for Health Care Assistants in theteam. Able to undertake annual PDPs if required .Recognises the development needsof others and provides clinical supervision. Actively participates in clinicaland practice meetings. Develops own knowledge and practice to meetobjectives/changes in service.
Key role - influence and develop policy -Create and capitalise uponopportunities to influence the setting of policies which will affect health andsocial well being.Develops and maintains clinical protocols for the surgery andthe team, ensuring they are reviewed and approved annually.
Key role - manage resources -Monitors and uses resources appropriatelywithin the team e.g. dressings, vaccines, stationary. Ensures stock rotation inown and doctors clinical rooms.Undertake stock ordering on a weekly basis,being mindful of budget, and fluctuations in service provision.
Key role - manage and use information -Ensures own IT skills aresufficient to enable use of the practice IT systems to undertake own audit andrecall programs.Makes effective use of the clinical system by ensuring theconsistent and accurate entering of data and use of Read Codes.Facilitates theuse of up to date information technology in order to collate accurate andtimely information as and when required by the practice and ICB. Makesrecommendations and initiates raising of practice standards and servicedelivery.Contributes to a satisfactory call and recall system of patients, andundertakes own searches
Key role - research and development -Contributes to ICB/Service/Teamaudit/Research activity as required.Present audit findings at practice meetingswhen requested.Identifies, applies and disseminates research findings relatingto specialist practice.
Key role - Administration and general duties -Attend practice and nursemeetings as and when required by the practice manager. Undertakes ordering ofsupplies as required. Ensures supplies are stored in a safe, and cleanenvironment. With cupboards locked, and potentially dangerous equipment,chemicals and medicines out of the way of patients and visitors. Ensures allcare is documented in accordance with practice, ICB, NHS England protocols.Undertake routine checks on equipment as required by practice protocols.Ensures own work area is kept clean, tidy, and of professional appearance inorder to facilitate effective working. Ensure waste is disposed of inaccordance with practice Infection control policy.

* Qualified RGN or equivalent with current NMC registration
* Good standard of general education and good standard of spoken and written english language.
