About the Role
Waythrough is proud to be part of Reconnected to Health, delivering drug and alcohol recovery services across all seven North East prison establishments. Working in partnership with Spectrum Community Health CIC and Tees, Esk and Wear Valleys NHS Trust, we provide holistic support to men and women in custody across the region.
Within the prison healthcare system, we deliver recovery co-ordination, psychosocial interventions, and continuity of care planning. We work closely with community services nationwide and have strong links with North East drug and alcohol services.
As a Reconnect Navigator, you will support individuals preparing to leave prison by helping them overcome health-related barriers, build positive community connections, and access the support needed to maintain recovery and improve wellbeing.
Key Responsibilities
1. Manage a caseload of service users preparing for release from prison
2. Develop person-centred recovery plans
3. Support access to healthcare, recovery, and community services
4. Work collaboratively with prisons, probation, healthcare teams, and community organisations
5. Promote continuity of care and positive recovery outcomes
6. Maintain accurate records and follow organisational and prison procedures
7. Contribute to a positive and recovery-focused environment
Working Hours
This role is Monday to Friday, 08:00–16:00. There may occasionally be a requirement to work evenings or weekends depending on service needs. Any additional hours worked would be given back during the working week.

8. Full valid UK driving licence
9. Access to your own vehicle
What We Are Looking For
Essential Qualifications and Skills
10. NVQ Level 3 in Health and Social Care or equivalent (or willingness to work towards)
11. Proficient in Microsoft Office programmes
12. Ability to complete assessments, recovery plans, and risk management plans
Knowledge and Experience
13. Understanding of interventions including MI, PSIs, CBT, ITEP, and brief interventions
14. Knowledge of substance misuse, recovery models, and recovery planning
15. Experience supporting individuals with complex needs
16. Understanding of safeguarding and risk management
17. Ability to work effectively with partner agencies and stakeholders
