The role

Derbyshire Dales PCN (DDPCN) is a forward-thinking primary carenetwork seeking to embrace the opportunity to work with an established ClinicalPharmacist who will be working directly for one of our stakeholder practices (Matlock & Ashover Group Surgery). This is a two site GP Practice operating from Matlock and the village of Ashover, Derbyshire.

The successful candidate will work at and be under the direction of the practice team who are looking to employ an established Clinical Pharmacist preferably with experience of dispensary working to join the Matlock and Ashover team.

DDPCN is committed to supporting the successful candidate in their career in this particular general practice where you will have opportunities to enhance your skills in a primary setting which will include the support of our experienced Lead Pharmacist and wider Practice clinical teams.

Main duties of the job

To work as part of a multi-disciplinary team in a patient-facing role taking responsibility for areas of chronic disease management within the practice.

To undertake structured medication reviews to proactively manage patients with complex polypharmacy, especially for older people and those with multiple co-morbidities.

To provide support to general practice staff with regards to prescription and medication queries.

To help support the repeat prescriptions system and deal with acute prescription requests.

To undertake medicines reconciliation on transfer of care and contribute to the development of systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the GP practice(s).

To proactively work alongside the wider PCN, MSK and Mental Health multidisciplinary teams, on projects such as tackling health inequalities, low prevalence for CVD and high levels of opiates.

The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.

About us

DDPCN is based in Tansley, Matlock and supports and works in partnership with all the GP Practices in the Derbyshire Dales.

Whilst the PCN office is in the heart of the Derbyshire Dales, your day-to-day duties will be at the two site premises that are run by the Matlock and Ashover Group Practice.

There is excellent road infrastructure with access to the M1 Motorway, Sheffield, Derby, towards Manchester and Staffordshire are easily commutable.

You will be joining a friendly, forward-thinking practice that supports approximately 10,000 patients, delivering high quality patient-focused care.

Job responsibilities

To work as part of a multi-disciplinary practice team in a patient-facing role taking responsibility for areas of chronic disease management within the practice.

To provide support to general practice staff with regards to prescription and medication queries.

As the Clinical Pharmacist you will work as part of a multi-disciplinary team in a patient-facing role to clinically assess and treat patients using their expert knowledge of medicines.

The ideal candidate will be an experienced prescriber, or will be completing training to become a prescriber, and will work with and alongside the general practice team.

As the Clinical Pharmacists you will undertake medication reviews to proactively manage people with LTC, people in care homes, frailty, and people with learning disabilities or autism.

Clinical Pharmacists will provide specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients at the practice and to help in tackling inequalities.

Clinical Pharmacists will provide leadership on person-centred medicines optimisation (including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement, whilst contributing to the QOF and both practice /PCN enhanced services.

Through structured medication reviews, clinical pharmacists will support patients to take their medications to get the best from them, reduce waste and promote self-care.

Clinical Pharmacists will have a leadership role in supporting further integration of general practice with the wider healthcare teams (including community and hospital pharmacy) to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload.

The role has the potential to significantly improve quality of care and safety for patients.

The Clinical Pharmacist will develop relationships and work closely with other pharmacy professionals across DDPCN, Derbyshire Dales and the wider health care systems.

Clinical Pharmacists will take a central role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists (including mental health and reduction of inappropriate antipsychotic use in people with learning difficulties), liaison with community pharmacists and anticoagulation.

Take part in Clinical Audit and quality improvement to ensure prescribing is in line with local and national guidance, CQC requirements and up to date MHRA /CAS alerts.

Clinical Pharmacists as appropriate will support the Lead Clinical Pharmacist in mentoring providing training /support for the Pharmacy Technicians and Pharmacy Administrators.

Support the wider clinical team with education and induction programmes for new clinical staff.

Support the Lead Clinical Pharmacist in ensuring appropriate representation at local ICB Medicine Management Team meetings and other PCN/practice meetings.

Person Specification

Qualifications

  • Master's degree in pharmacy
  • Audit and Quality Improvement
  • Proficient in use of Microsoft Office (including Excel, Word, PowerPoint, Teams and Outlook)
  • Community or primary care / NHS experience.
  • Full Drivers Licence and be able to travel independently to the two practice sites whenever required.
  • Independent Prescriber (IP) or working towards qualification.
  • Be willing to undertake IP course if required as part of role requirement.
  • MDT Working.
  • Experience of dispensary working /management
  • Knowledge and experience of using SystmOne
  • Developing pathways or processes
  • Understanding of QOF, IIF and PCN DES processes
  • MHRA and CAS alerts
  • Understanding of Drug Monitoring and local relevant clinical and national guidance
  • Training and Mentoring experience.
