The Family Welfare Coach supports families facing complex challenges by building strong, trusting relationships and walking alongside them to improve wellbeing, confidence, and independence. Using a trauma-informed and culturally responsive approach, you will offer holistic coaching support, help families navigate services, and connect them to social prescribing pathways and community-led solutions. The role will:
* Co-ordinate, simplify and broker access to a range of health and social interventions based on the needs and wishes of the individual, including liaising with a range of agencies (schools, housing, Family Intervention Service - FIS)
* Empower the individual to express their views, wishes and concerns
* Co-produce a personalised support plan that encourages the empowerment of an individual to take control of their own health and wellbeing (‘what matters to me’)
* Facilitate delivery of a peer led ‘mindful mamas’ group
* Connect individuals to local and city-wide services
* Develop close working partnerships with health, social care, housing and voluntary, community or social enterprise (VCSE) organisations.
