The homecare medicine service coordinates the supply of medication to over 4000 patients with long term conditions. Main duties of the job
To assist the Pharmacy Homecare Senior Technician and Lead Homecare Pharmacists throughout the homecare pathway, in safe and cost-effective procurement and supply of medicine to patients via third party homecare companies for Hampshire Hospitals NHS Foundation Trust (HHFT).
To ensure all pharmacy duties are carried out in accordance with legal guidelines and departmental and Trust procedures.
To support in the planning and implementing of projects such as biosimilar switches and contract implementation to ensure homecare services follow cost effective procurement.
To act upon and manage any price queries or problems, investigating and resolving discrepancies and ensuring invoices are processed in a timely manner.
Undertake regular operational commitment to maintain competencies and demonstrate fitness to practice as a pharmacy technician. To undertake any other duties applicable to the grade/post as required.

Detailed job description and main responsibilities
To support throughout the homecare pathway, from reviewing appropriateness of registering
homecare patients, through to final processing of invoices.
• Resolve issues including price discrepancies, non-delivery issues, late deliveries, change of
address, contract issues and updating of all relevant databases.
• Work in line with the Royal Pharmaceutical Society Standards for Homecare & the Hackett report.
• Assist in coordinating the pharmacy homecare medicines service, prioritise and manage own
workload and resolve problems as they arise.
• To assist the Homecare Leads to develop services for the safe and timely supply of medicines to
patients by establishing robust methods of managing the workflow.
• Review and maintain departmental & third-party Key Performance Indicators (KPIs) and provide
feedback in service review meetings for homecare services.
• To act as the point of contact between the medicines home delivery service for patients, clinical
teams, and homecare providers.
• To put into practice all pharmacy policies and standard operating procedures (SOPs) and to assist
the Pharmacy Homecare Leads in the governance through policy and protocol development.
• To be aware of the Medicines Act, Health and Safety at Work Act, COSHH and their implications
for work within the department to ensure a lawful and safe working environment. To actively
participate in Risk Management within the Trust.
• Undertake any other reasonable duties consistent with grade, as requested by line manager or
Senior Management.
