Job Description
We are currently recruiting for someone to join our team as a Care Coordinator & Scheduler to provide efficient scheduling with the coordination and support of care delivery, ensuring continuity, quality, and compassion for both our clients and our Care Professionals.
This role will be carried out on a full time basis at our CQC Outstanding Chelford office near Macclesfield/Alderley Edge, working Monday-Friday 9am - 5pm with additional out of hours working as part of an on-call rota.
We are offering a salary of £28,000-£30,000. In addition, on-call duties are paid at £350 per week (for a full week) and you will be part of the monthly bonus scheme.
Responsibilities:
* Build, manage, and maintain effective schedules that meet client needs and preferences while optimising Care Professional availability.
* Match Care Professionals with clients thoughtfully, supporting relationship-building through consistent visits and effective introductions.
* Proactively adapt to changes in schedules due to client needs, staff availability, or unforeseen circumstances, working closely with the wider care team to implement solutions quickly.
* Maintain accurate and up-to-date records for both clients and Care Professionals using the electronic scheduling system, supporting compliance and reporting requirements.
* Act as a key contact for clients and Care Professionals, handling scheduling queries, care concerns, and service updates with professionalism and empathy.
* Support the onboarding and induction of new Care Professionals, helping to coordinate shadowing, training, and first visits.
* Communicate regularly with clients and their families to review care arrangements and ensure satisfaction, escalating issues where needed.
* Collaborate with the Registered Care Manager, Client Services Manager, and Quality Team to ensure service delivery aligns with CQC standards and internal quality expectations.
* Contribute to quality assurance activities, such as feedback calls, reviews, and documentation checks, to support the delivery of outstanding care.
* Support with on-call duties as part of a shared rota, responding to out-of-hours needs with confidence and calm decision-making.
