Care Assistant – Community Recovery Services Summary of role As a Care Assistant within our Community Recovery Services team, you will play a vital role in supporting individuals to regain confidence, independence, and daily living skills following illness, injury, or hospital admission. Working within the community, you will provide person-centred care that promotes dignity, choice, and recovery. This role involves supporting people in their own homes to achieve agreed outcomes and rebuild independence, rather than creating long-term dependency Principal responsibilities To assist individuals with personal care tasks, including washing, dressing, toileting, and maintaining hygiene in a respectful and dignified manner. To support clients with mobility, including the safe use of aids, equipment, and rehabilitation techniques as directed by health professionals. To promote independence through encouragement, motivation, and active participation in daily routines such as meal preparation, household tasks, and personal goal setting. To provide support with medication prompts in accordance with individual care plans. To observe, record, and report any changes in an individual’s health, wellbeing, or circumstances to the Recovery Support Coordinator or Community Team Lead. To work collaboratively with occupational therapists, physiotherapists, and nurses to deliver personalised recovery plans. To encourage engagement in social and therapeutic activities that support emotional and physical wellbeing. To assist with light domestic duties where required, ensuring a safe and comfortable home environment. To maintain accurate records and contribute to reviews, ensuring that care remains responsive and effective. To adhere to all company policies, procedures, and safeguarding standards at all times.
