More About Your Role
* Set up and manage a range of sustainable Healthy Communities projects/programmes that deliver measurable impact for the community of Amber Valley. Focus on tackling health inequalities while collaborating with the Area Healthy Communities manager to create, implement, and review the Places Leisure Amber Valley Healthy Communities plan reflecting the needs of local communities.
* Work closely with local stakeholders including the Active Partnership and the Move More Amber Valley network to strategically develop, manage and develop the Exercise Referral programme, ensuring effective pathways and development of system processes to capture impact and change. Include the development and expansion of the community model of health referral across Amber Valley.
* Build and maintain relationships with GP Practices, Active Derbyshire, Amber Valley Borough Council, Public Health, Primary Care Networks and community partners.
* Work collaboratively across the local area to enhance community access and social impact.
More About You
You will be an active member of key health and wellbeing partner groups related to Active Partnerships, the NHS, the Local Authority, VCSE, etc. We are also looking for someone who has multiple qualifications in a range of disciplines such as Level 3 Personal Trainer, Level 3 Exercise Referral, Level 4 Gym, Level 2 NGB Coaching, Bachelor’s Degree, Master’s or equivalent experience.
