Overview
The post holder is a senior member of the Inpatient Mental Health Team and will be responsible for the care and care coordination for a caseload of service users experiencing severe mental health difficulties. They will take responsibility for assessment of health and social needs, formulation, planning, delivery and evaluation of care, support and treatment by the team for individuals and groups, including risk assessment and management, CTP and CTP reviews.
Responsibilities
* Complete CTP assessments for patients requiring out-of-hours assessment; competent in CTP/risk assessment, formulation and planning outcome of assessment.
* Supervise junior staff engaged with service users, complete junior staff appraisals, act as preceptor and mentor to newly qualified staff and nursing students; assume line management in the absence of the ward manager.
* Collaborate across wide-ranging multidisciplinary services and agencies to provide effective delivery of health and social care, and support a programme of recovery and social inclusion within the area.
* Supervise care planning and ensure delivery, monitoring and evaluation of care and treatment plans; maintain and improve clinical governance.
* Lead on risk assessment and management for caseloads and oversee CTP reviews.
Person Specification
Qualifications
* Registered Mental Health Nurse – Degree level or working towards
* Nurse Prescribing – or willing to complete
* Training in CBT in Psychosis / DBT training
Desirable
* ECDL
Knowledge
* Knowledge and experience of WARRN risk assessment and assessments
* Evidence of Continuous Professional Development
* Knowledge of psychosocial interventions CBT and principles of recovery within a mental health setting
* Administration, monitoring and advising on medication
* Knowledge of Mental Health Act 1983
* Understanding of culture, social and health beliefs and commitment to service user involvement and empowerment
* Clinical governance and quality assurance
* Mentorship/preceptorship
Experience
* Significant experience at Band 5 level
* Experience supervising and developing junior staff; leading teams
* Experience leading change or new service development
* Clinical supervision and involvement in clinical audits
* Experience of CTP/risk assessment/formulation/planning and implementing care plans
* Experience in health promotion

* Excellent organisational skills
