12 Fixed Term Contract
Part time (22.5 hours per week, weekdays only)
St Lukes Hospice is looking for an Integrated Care Facilitator, who has experience working within health and social care, who has a passion to use this knowledge to enhance the lives of our Palliative care patients and their significant others.
As their Integrated Care Facilitator, you will use your specialist knowledge of the health and social care system to support your clinical colleagues within the Hospice, primary care and wider healthcare providers. You will be working directly with patients and their significant others in the communities where they live, helping to develop a deep understanding of the many local initiatives which can support them in a more creative way.
You will be pivotal in helping us develop and evaluate this exciting role, and your views and experience will be critical in helping us be the very best at what we do. We recognise the critical impact and challenges current health and social care provision has on our patients and their significant others, and we are looking for someone who can maximise the support and choice for our patients, from referral through to end of life.
We are looking for someone who uses their own imitative and is able to work as part of a team, who can provide support across our 3 community hubs.
Experience of working within the clinical setting and/or specialist palliative care would be an advantage but not essential and training will be given to the right individuals. Knowledge of care provision across Plymouth and South Hams and West Devon would be a particular advantage.
We want our team to represent the diverse community we serve and have a strong track record in supporting flexible working and developing people. As a staff member of St Luke's you will receive an attractive employment package, which includes 7 weeks annual leave rising with service (inc. bank hols) and membership to a cash back health plan.
We would encourage informal discussions regarding this post with our Clinical Lead or Deputy Clinical Leads for End of Life Urgent Care Service: These include people from Black, Asian and minority ethnic backgrounds, disabled people and LGBTQ+ people.
Not only generous annual leave and pension schemes, but a fantastic health scheme to help towards the cost of eye tests, dental care and physiotherapy. In addition, this gives you access a GP, and counselling and practical information helplines, 24/7.
While working with our hospice is hugely rewarding, knowing you're helping us make such an important difference, we recognise it's work that can be challenging emotionally. Generous Annual Leave
