Job overview

The role is a key clinical role focused on delivering Physiotherapy or Occupational Therapy and case management for patients with complex needs within community discharge hubs, interim health-funded beds, and inpatient setting on Chartridge Ward. The postholder will provide holistic assessment, discharge planning, and collaborative care, ensuring timely and safe transitions of care. They will also support the wider multi-disciplinary team (MDT) and support senior staff when required.

Main duties of the job

·To participate within a clinical and or interdisciplinary team and contribute to it’s function.

·Provide therapy input across all Community Discharge Hubs, Interim Health Funded (IHF) beds, BHT hospitals, and other locality-based inpatient settings to support the delivery of a responsive and efficient discharge hub service.

·Provide holistic assessments and case management for patients with complex health and rehabilitation needs, ensuring accurate identification of ongoing care and therapy requirements.

·Help case manage patients within complex health and rehabilitation needs in designated bed bases, creating and implementing person-centred discharge plans that optimise outcomes and reduce readmission risk.

·Hold responsibility for a defined caseload of patients with highly complex needs.

·Act as a resource and educator for members of the multi-disciplinary team (MDT).

·Engage and collaborate with external partners and agencies (e.g., social care, voluntary sector, primary care) to ensure smooth and safe discharge planning and to prevent failed or delayed discharges.

Utilise digital tools and information systems to support patient tracking, case finding, and early intervention for individuals at risk of poor outcomes.

·Advocate for individuals with long-term conditions, promoting coordinated, person-centred care that supports independence, recovery, and quality of life.

EDUCATION, QUALIFICATIONS & TRAINING

Essential criteria

6. Registered Allied Healthcare Professional with HCPC - Physiotherapist Occupational Therapist
7. Degree/ Diploma in Physiotherapy Occupational Therapy
8. Experience of recent Continuous Professional Development.
9. Relevant post graduate clinical training e.g. Advanced History Taking & Assessment for example; Falls, Dementia and Delirium, Long Term Conditions, Community, Orthopaedics

EXPERIENCE

Essential criteria

10. Demonstrate experience post or pre-registration placements in relevant clinical areas for therapists.
11. Experience of working in a community/in-patient setting for a minimum of 2 years post registration.
12. In depth experience of working within multi-agency/multi-disciplinary team.
13. Experience of working with single assessment process.
14. Experience of electronic clinical systems.
15. Experience participating in research or audit project.
16. Experience of day-to-day staff management e.g. case allocation to Health Care Assistants

SKILLS, ABILITIES & KNOWLEDGE

17. Demonstrate profession specific clinical assessment planning delivery and evaluation skills.
18. Ability to manage complex cases.
19. Ability to develop and maintain partnership working.
20. Knowledge of legal requirement for working with vulnerable adults and those who lack capacity.

22. Knowledge of modernisation agenda within the NHS and social care.
23. Excellent written and verbal communication skills.
