Job Description: Senior Practice Nurse
Provide a holistic approach to patients, including assessment, diagnosis, planning, and implementation of high-quality care within the scope of chronic disease management.
Work collaboratively within the team to meet patient needs using evidence-based care principles. Liaise with the Nursing & AHP Clinical Lead to ensure required audits are completed and all practice staff comply with local and national policies.

The Senior Practice Nurse will work as part of a multi-disciplinary team, responding to referrals from GPs, community nurses, and other health & social care colleagues.
This role requires extensive autonomous practice, including independent clinical decision-making, critical thinking, and diagnostic reasoning within the scope of chronic disease management.
Key responsibilities include:
1. Advanced clinical nursing skills and patient assessment
2. Leading complex care coordination
3. Managing complex long-term conditions within the practice
4. Infection control management and leadership
5. Supporting self-care and promoting independence
6. Identifying high-risk individuals and promoting health and disease prevention
Northumbria Primary Care Ltd (NPC) manages several GP practices across Northumberland and North Tyneside, serving approximately 120,000 patients. While appointees will have a base practice, they may work across other practices within the group, with efforts made to maintain continuity of care.

Additional Responsibilities:
* Serve as the Nursing Lead within the Practice, providing senior leadership and support to the Nursing and Healthcare Assistant teams.
* Deliver a needs-led service aligned with the Practice Strategy and NHS modernization agenda.
* Ensure adherence to professional standards, policies, and procedures.
* Operate autonomously as lead nurse for Chronic Disease Management, maintaining advanced clinical skills through formal qualifications, including non-medical prescribing.
* Provide holistic patient care, including assessment, diagnosis, planning, and implementation within the chronic disease scope.
* Lead infection control initiatives and ensure audit compliance.
* Support training and education for staff and students, fostering a learning environment.
* Work independently, demonstrating critical thinking and clinical decision-making skills.
* Assist in clinical supervision and staff appraisals.
* Model compassionate, inclusive leadership to promote a culture of shared leadership, engagement, and collaboration.
