HCP 0-19 Public Health Practitioner

Location: Fenland area of the North Locality (Wisbech and Doddington)
Role for fully qualified or newly qualified Registered Nurse to deliver the 0-19 Healthy Child Programme across Cambridgeshire and Peterborough. The post requires a current NMC registration and relevant experience.
Main duties and responsibilities
* Provide a clinically effective high‑quality service to children, young people and families under the direction of the HCP Team Leader.
* Work within a corporate caseload with other team members to assist in the provision of family‑centred public health service and preserve the health of individuals, families, and communities.
* Support the delivery of the 4,5,6 model of care within the HCP, delivering universal services, and meet complex needs while supporting the SCPHN in improving outcomes, especially in the six high‑impact areas.
* Work in partnership with key agencies such as Children’s Services and Education to address health inequalities and improve outcomes for families.
* Undertake appropriate further academic qualifications within 2 years of appointment to enable progression to Specialist Community Public Health Nursing (subject to ability and availability).
* Manage own caseload for identified children and young people and deliver care in settings such as schools, clinics, client homes, and community venues.
* Deliver Universal New Born contacts, 6‑week checks, 1‑ and 2‑year reviews, transfer‑in contacts, and school health assessments where competency is demonstrated.
* Attend events to promote the service, review records of children moving in/out of the area, and assist in the delivery of Public Health group activities.
* Report concerns to the HCP 0‑19 Team Leader or Locality SCPHN and adhere to safeguarding and child‑protection procedures.
* Act as a practice supervisor or practice assessor to student nurses following approved training.
* Support Children, Families and Young People to ensure their voices are heard within service development, including the use of digital technologies and online resources.
* Assist the Duty Clinician with the delivery of Text Us, ChatHealth, and Duty Desk, and support the digital service offer.
* Lead child health clinics and promote effective communication with children, young people, parents, carers and other professionals in complex situations.
Clinical and Practice Governance
* Maintain current NMC registration and professional registration throughout employment.
* Observe strict confidentiality and data protection principles, and comply with Trust Information Governance and Information Security policies.
* Follow Trust risk assessment, risk management, infection control policies, and mandatory training requirements.
* Participate in clinical and safeguarding audits and emergency preparedness processes.
* Provide patients and families with information on standards expected from the team.

Skills
* Ability to manage and prioritise workload.
* Excellent written and verbal communication skills.
Qualifications & Training
* First level Registered Nurse (General, Child, Mental Health or Learning Disabilities) or Midwife.
* First level degree or equivalent.
* Knowledge of the role of the Healthy Child Programme practitioner in family‑centred Public Health.
* Experience of supporting and supervising students in a Practice or Assessor role.
* Experience of academic study at level 6 within the last 3 years.
Experience
* Experience working in a multidisciplinary setting.
* Experience of safeguarding adults and/or children.
* Current knowledge of local and national policies ensuring the delivery of the Healthy Child Programme.
* Experience of mentoring.
* Experience working in the community and with children and their families.
* Experience managing or leading staff.

Salary: £32,073 to £39,043 a year pro‑rata.
