and main responsibilities
1. To coordinate discharge referrals from UHD ensuring the team have all the required information and ensuring the correct procedures have been followed.
2. To be involved in administrative tasks relevant to the interim care team.
3. Work closely with hospital therapists, the social work teams and discharge keyworkers to deliver high quality, efficient and effective discharges.
4. To coordinate the therapy assistant’s timetable ensuring there is adequate time for care visits, travel and breaks.
5. To produce accurate reports in accordance with strict deadlines

Qualifications
Essential criteria
6. NVQ III Care or equivalent
Experience
Essential criteria
7. Recent experience working in a health or social care setting
8. Competent using a large range of IT software packages e.g. Microsoft Office; excel; outlook
Desirable criteria
9. Previous experience in the NHS
10. Previous experience working as a therapy assistant or carer

12. Ability to use own initiative within sphere of responsibility
13. Ability to effectively organise and prioritise own workload and ensure deadlines are met with supervision
