Post Covid Service Respiratory Physiotherapist Band 7
Support Team Lead in the development and maintenance of policies and procedures within the Post-COVID Assessment Service. Lead where required on the development and maintenance of processes and pathways for Post-COVID Assessment Service. To review referrals, support triage and liaison for patients referred into Post-COVID Assessment Service and the airways team. Develop positive and professional relationships across a wide range of internal and external groups, agencies and service providers. Supporting the MDT process and lead where required at Post-COVID Assessment Service, review patients holistic needs, communicating highly complex and sensitive information to referrers, and colleagues, patient and carers as required. Where there are barriers to understanding, such as cultural barriers, hearing impairment, mental capacity impairment and other difficulties in comprehension or language impairments; awareness of reasonable adjustments. Use risk stratification methods in partnership with other professionals to identify those patients most likely at risk of health, social care and wellbeing crisis and focus support to ensure the patients independence is maximised. Identification and awareness of inequalities in health outcomes and access to support for Norfolk and Waveney citizens. Be required to use tact and persuasive skills in order to coordinate and plan on-going care, treatment or management with colleagues, for patients. Liaise with GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care co-ordinated appropriately. To participate and lead (where appropriate) in multi-disciplinary / multi-agency meetings as appropriate. Provide clinical expertise and advice to others, including social services, GPs and other agencies. Ensure activity data and clinical information is recorded on system one and other tools as required, to be completed on a daily basis. Demonstrate dexterity and co-ordination when using specialist equipment and fine tools, advances sensory skills, manual and mobilising skills.
