The primary focus of the Senior Primary Mental Health Practitioner will be to support high quality service delivery of the Primary Care Mental Health Service across a range of practices within their designated PCN and lead on the co-ordination of the PCN’s SMI registers. The post holder will be required to engage a range of key partners and stakeholders and lead on joint working across health and social care. They will also work alongside the Primary Care Networks to support them in meeting their priorities around prevention, reducing health inequalities and population health management.). The role will provide an opportunity for improved joint working across primary care and adult secondary care mental health systems and will make it easier for patients to obtain the help they need.

Main duties of the job
To provide a lead role based in the ‘Wolverhampton Total Health” PCN in taking the lead in the day to day delivery of a high quality Primary Care Mental Health service, managing resources and conflicting priorities.
To review patients presenting with mental health issues, consider a range of options regarding treatment interventions providing them with highly specialised advice concerning care when appropriate and liaising with GPs and MH teams.
To offer brief interventions as needed e.g. problem solving, decision making, building resilience, building on personal strengths, risk de-escalation, medication advice and prescribing if a registered NMP. (Training will be provided as required).
Working as a member of the Primary Care MDT and taking responsibility for the development and implementation of specialist mental health training to the PCN.
To help GPs make appropriate referrals to the relevant mental health services - bridging the gap between primary care services such as IAPT and Primary Care Mental Health, working in partnership across the system.

Detailed job description and main responsibilities
As a Senior Primary Care Mental Health practitioner, the post holder will work as part of a dynamic team in delivering an effective service supporting Primary Care Networks (PCN’s) in Wolverhampton.
The primary focus of the post holder will be to support high quality service delivery of the Primary Care Mental Health Service across a range of practices within their designated PCN and lead on the co-ordination of the PCN’s SMI registers. In particular, they will need to work closely with colleagues within Black Country healthcare Mental Health Services, the PCN’s, other provider organisations, local authority/Office of Public Health, and the voluntary and community sector. They will also work alongside the Primary Care Networks to support them in meeting their priorities around prevention, reducing health inequalities and population health management.

* Degree or equivalent qualification or experience in evidence based psychological therapies relevant to Pathway.

The salary range quoted is as per NHS Agenda for Change pay structures. This is based on the number of years’ working directly within the NHS at the advertised pay band. Experience gained with other NHS Trusts will apply, but candidates without prior experience working direct for the NHS would normally be appointed at the lowest point unless they can demonstrate the equivalent experience and then be eligible for incremental increases in future years.
Successful applicants for clinical posts up to and including Band 4 are expected to hold the national Care Certificate qualification and will be supported to attain this once in post.
