We have an excellent career opportunity for the role of Care Co-Ordinator to join our amazing team here at Tamar Homecare based in Plymouth.

Pay:

* An initial salary of £30,000, per annum with minimum contracted hours of 40 hours per week.

Key responsibilities:

* Coordinating care services and allocating care staff to meet the needs of our Service Users
* Producing efficient weekly rotas and ensuring continuity of care
* Communicating effectively with Service Users, care staff, social workers, and other stakeholders
* Maintaining accurate client and staff records
* Planning capacity and assisting with new client referrals
* Supporting with recruitment needs, quality assurance, training records, and performance monitoring
* Managing holiday, absence and emergency cover
* Attending management meetings and contributing to service development

Essential criteria:

* Previous experience in care coordination or scheduling within a domiciliary care setting
* Strong organisational skills and the ability to multitask in a fast-paced environment
* Excellent communication skills and a caring, customer-focused attitude
* Good IT skills and confidence using scheduling or care management software
* Knowledge of CQC regulations and care industry best practices (desirable)
* Willing to be a team player.
* Professional manner at all times
* Friendly attitude towards all staff and clients.
* Your own car.
* NVQ Level 3 or equivalent in Health and Social Care
* Able to work alternative weekends (this makes it fair for all staff).

Job Types: Full-time, Permanent

Pay: Up to £30,000.00 per year
