An opportunity has arisen within the Planned Care Division for a SMART Vascular Pathway Coordinator within the Vascular department. Accountable to the Assistant Service Manager, the Vascular Pathway Coordinator Manager will provide support to the Assistant Service Manager for the operational delivery of Vascular Services.
Main duties of the job
The post holder will be responsible for:
· Tracking the progress of Vascular Surgery patients through the care and core services of the SMART Network.
· Ensuring that the relevant information for the SMART Network Multidisciplinary Team (MDT) meeting is available to facilitate the provision of care and core services.
· To monitor and manage Vascular Surgery waiting lists for the SMART Network in line with SMART Network policies and procedure, maximising theatre utilisation.
· Liaising with departments involved in providing care and support services throughout the SMART Network to ensure that planned patient treatment progresses smoothly and in a timely manner.
