The post will be instrumental in the success and future developments of the pathway and will ensure that all professional and patient contact with the service is of the highest professional standard at all times. Working alongside senior clinical staff, you will respond rapidly to requests for assistance when a young person is presenting in crisis.
You will contribute to the assessment and management of young people experiencing episodes of crisis within a clear and structured Risk Assessment and Risk Reduction framework and identify therapeutic goals to support the young person and families/carers.
You will refer on to specialist in-patient services as necessary or work collaboratively with other services involved to reduce the need for inpatient care. You will develop holistic, safety and recovery-focused care plans and mobilise support around the child and family to promote resilience and reduce future crisis episodes.
As a member of the multi-disciplinary team, the service will support young people and families through various pathways, including in-reach, consultation and assessment models, out-of-hours Crisis Access and Referral Point, rapid intervention on 72-hour care pathways, and longer-term intervention on 4-6 week Home Intensive Treatment care pathways.
Key Responsibilities
1. Manage complex caseloads with high professional responsibility for assessment, treatment, and discharge of children, young people, and their families, collaborating with relevant agencies.
2. Conduct specialist assessments of CYP with mental health needs, including risk assessment, gathering information from families and other agencies, and developing intervention plans in collaboration with the child, young person, and their carers.
3. Deliver evidence-based interventions aligned with NICE guidance, supported by highly developed specialist knowledge, and meet the standards of recommended psychological therapies.
4. Supervise junior team members, providing guidance and fostering a collaborative environment. Support staff training and continuous service improvement.
5. Complete audits to ensure compliance with clinical governance frameworks and regulations, and contribute to developing new clinical services and initiatives.
