The South Community Mental Health Service is undergoing an exciting transformation with additional resource allocation to provide ongoing intensive support to the population of Sheffield. This includes working towards a joint approach with Primary Care and the Voluntary Sector, reviewing the CMHT Service Criteria, and working in small teams within local communities to uphold high-quality care standards.
We are looking to recruit a Band 6 Nurse or Social Worker to the South CMHT.
The successful candidate will be supported with their professional and leadership development through dedicated supervision with experienced team managers. Learning opportunities will be encouraged, including regular CPD sessions where candidates can bring their interests to share and develop within mental health. Please note, this is a qualified Nursing vacancy.
You will need excellent communication skills and the ability to work independently and as part of a team to assess, plan, and evaluate care. A positive approach to meeting service users' needs, along with motivation, empowerment, and enthusiasm to learn and share, are essential.
You will also be expected to work as part of a duty system operating within the hours of 09:00 to 17:00, Monday to Friday, on a rotational basis. This 'mini team' is supported by other clinical professionals and management.

The post holder will work within a multidisciplinary team, with core responsibilities including:
1. Acting as a Named Worker
2. Conducting comprehensive assessments of new referrals
3. Managing a varied caseload based on need
4. Participating in a duty system on a rota basis
5. Providing supervision to junior staff
6. Contributing to multidisciplinary service development
Clinical responsibilities include:
1. Triage and determine the urgency of new referrals
2. Assessing individuals’ health and social care needs in various settings and deciding on appropriate actions, including first presentation to the service
3. Assessing, evaluating, and managing clinical risk
4. Formulating care plans based on identified needs
5. Delivering evidence-based interventions as appropriate
6. Reviewing care plan effectiveness regularly
7. Working within safeguarding processes, including acting as a safeguarding investigator/manager when required
8. Collaborating with service users, carers, and partner agencies
9. Maintaining accurate and timely computer records
10. Undertaking all work in a culturally sensitive manner
11. Adhering to Care Management System, Acute, and Schedule Care Pathways as appropriate
12. Developing effective working relationships with Primary Care colleagues
