We have a 2-year secondment or fixed-term post for a forward-thinking, dynamic senior nurse manager who would like to embrace this opportunity as Clinical Service Manager, within our LDA Enhanced Support & Liaison Service. Our Enhanced Support and Liaison Service went live in 2024, it is a county wide, 7 day per week service which functions as part of a multi-agency pathway, working with service users, providers, carers, families, social care, and health partners to enable people with a learning disability and/or autism in Lincolnshire to receive appropriate care in the right place at the right time. LDA Enhanced Support & Liaison Service offers the following levels of support: Level 1 – Generic support Level 2 – Patient Specific Support Level 3 – Enhanced Patient Specific Within the levels of intervention and support offered there are different pathways that an individual might access. These include: • Community Liaison Pathways • Inpatient/discharge Pathway • Enhanced and intensive community Intervention & Support It is the inpatient pathway that this Specialist Liaison Practitioner will be focused on. Advert You will provide direct operational management to The Enhanced Support and Liaison Service (ESLS). Key tasks include: • Be a visible and accessible leader, who will provide key senior clinical nursing leadership across the pathways, having oversight that those pathways are effective and efficient. Managing community based activity in line with locally set targets • Managing capacity to ensure that clinical priorities and waiting time targets are met through effective use of available resources. • Leading service modernisation projects with multidisciplinary teams. • Managing HR processes within designated services. • Leading on clinical governance actions such as complaint/incident investigations in conjunction with the Quality Assurance Lead. • To provide effective team leadership that will facilitate staff in ensuring that strong links with other providers of community/inpatient services are maintained and that staff utilise all available resources to meet the identified needs of individual patients. • To advise the senior management team on issues regarding finances, resources and staff. • To analyse data and recommend service improvements on the back of data analysis. • Critically analyse statistical information in collaboration with the Business Manager and clinical colleagues to ensure reliability, accuracy and consistency.
