This is for a Lead Nurse Role, working 25hrs per week (days to be arranged).
To manage the nursing team, providingvisible clinical and professional leadership to ensure a high standard ofgeneral practice nursing is delivered by working collaboratively with thegeneral practice team to meet the needs of patients.
The post holder willensure the delivery of exceptionally high-quality, safe, effective, efficient,and patient-centred general practice nursing services within the practice.Working collaboratively with the general practice team, the Lead Nurse willmeet the needs of patients, enhance service delivery, and contribute to thestrategic development of nursing services.
The post holder will carry outclinical sessions within their scope of practice. This key leadership roledemands advanced clinical expertise, strong team management skills, and theability to contribute to quality governance and service improvement.
Main duties of the job
Clinical Leadershipand Management
To lead, manage and deliver a range of practicenursing services, including long term condition management, health assessments,cytology and immunisations.
To be accountable for the nursing team on aday-to-day basis providing clinical leadership, mentorship and support toensure the delivery of safe, effective and patient-centred care.
Foster a learning environment, supporting thetraining and development of the nursing team, nursing and medical students,including clinical supervision where appropriate.
Lead the recruitment, induction, performancemonitoring, appraisals, and 1:1s for the nursing team.
About us
We are a very busy GP practice with 8 GP partners and 5 salaried GPs, with a patient list size of 20,000 patients, covering two sites. Our Nursing team consists of 8 Registered Nurses and 4 HCAs. We work closely with our neighbouring practice within the building as a PCN and have ARRS roles working with us including Clinical Pharmacists and Paramedics. The Nursing team work closely together within both the Truro Health Park and at our branch surgery at Threemilestone, running treatment room clinics alongside specialist long term condition clinics.

The role
To lead on ensuring the nursing services arecompliant with CQC and other regulatory authoritys requirements.
Leadquality improvement initiatives within the nursing service, oversee clinicalaudits, ensuring compliance with clinical guidelines and standards.
Contribute to the development, implementation,and review of practice policies, procedures, and protocols related to nursingand patient care and ensure these are up-to-date and used appropriately by theteam
Support the implementation and monitoring ofQOF, IIF and local incentive schemes.
To contribute ideas to the Senior LeadershipTeam regarding national working, strategy, and planning.
To support the clinical risk review activitiesand quality governance at Lander Medical Practice.
Collaboration and Communication
To work collaboratively with GPs, healthcareprofessionals and administrative staff to ensure integrated and coordinatedcare for patients
Represent the nursing team in practice meetingsand contribute to wider organisational objectives.
Develop and maintain effective communicationchannels within the nursing team and across practice sites.
Clinical
To see patients, face to face and performclinical assessment, diagnosis (where possible), and treatment (whereappropriate) for patients and refer to other clinicians as appropriate.
To maintain accurate computerised clinicalrecords of every contact, including history, examination, plan of care, advicegiven and recommended follow up. This includes documenting any adverse event,safeguarding child or vulnerable adult issue or other notifiable matter.
Assess, plan, implement and evaluate individualtreatment plans for patients with a known long-term condition.
Proactively identify and manage treatment plansfor patients at risk of developing a long-term condition as appropriate.
Work with other health care professionals tomonitor, manage and treat long-term conditions, including non-drug-basedtreatment methods using a management plan, and in line with national and localpolicies and to refer on as needed.
Review medication for therapeutic effectiveness,appropriate to patient needs and in accordance with evidence-based practice andnational and practice protocols.
Work with patients to support adherence toprescribed treatments.
Work within patient group directions/ patientspecific directions in collaboration with GPs and other primary care nurses tomeet the needs of patients.
Support patients to adopt health promotionstrategies that promote patients to live healthily and apply principles ofself-care.
Understand and apply legal policy that supportsthe identification of vulnerable and abused children and adults, being aware ofstatutory child/vulnerable adult health procedure and local guidance.
To be aware of and work within the Clinical Codeof Conduct and performance framework for the Care Quality Commission and theNational Quality Requirements.
Continue to develop, expand and role model ownpersonal clinical expertise.
Specialist Responsibilities Infection PreventionChampion
To undertake the Infection Prevention ChampionRole with the aim of helping create and maintain an environment which promotesand improves the safety of patients, service users, staff and visitors inrelation to infection prevention and control (IPC). To translate IPC policyinto practice and support and advise team/service area colleagues on IPCmatters.
To act as a resource and a point of contact forcolleagues who require support and guidance with IPC issues.
To cascade/disseminate IPC information receivedto colleagues within their teams.
Able to challenge poor practice and empowerothers to provide services safely
To maintain an awareness of the organisationspolicy and procedures in relation to IPC

* Registered General Nurse (RGN) qualification with valid NMC registration, with evidence of up-to-date practice.
