About the role

Are you looking for a change? A new challenge? The opportunity to develop new skills and knowledge in a new and exciting role. We need you for this amazing opportunity to work closely with Reablement and ITOCH, supporting and planning safe hospital discharges facilitating communication and coordination of our integrated services. Can you support Health Care Support Workers, act as a Key contact point, assess, plan and evaluate care, whilst delivering safe and effective care for our patients who are at the heart of what we do. This is a fabulous opportunity of a 6 month Secondment within our team.

We currently have a vacancy within Integrated Care Team: a service that provides 24 hour nursing care throughout the Wakefield Area. This post requires someone whom can be flexible with working patterns to support shift patterns, weekends and public holidays.

Working as part of the Integrated Care Team you will be providing care to patients that are managed by the Integrated Care Team caseload, this includes:

* Palliative care at home patients, supporting patients and families in their own home during their last days to weeks of life
* Rapid response patients that require support to stay in their own homes: preventing hospital admission, working closely with the Virtual Ward MDT keeping people at home and assisting in delivering care at home rather than in hospital.
* Rehabilitation patients: supporting them in regaining independence following an injury or illness, maintaining effective communication across services including Reablement
* Out of hours district nursing visits

We are looking for a highly motivated health care support worker who has a passion for delivering safe, high quality care to patients in the community setting

With a variety of clinical settings you would gain a wealth of knowledge and experience. We aim to support you in order for you to develop in your new role. We offer a fully comprehensive induction program and encourage all interested applicants to have an informal chat with staff in the field who can talk to you about the different roles.

Main duties

* To co-ordinate the efficient delivery of an extensive range of support services for a busy, district wide Community Intermediate Care Service covering diverse locations and groups of staff.
* To coordinate the delivery of care provided by Health Care Support Workers within the Community Intermediate Care Service
* To provide day to day support and supervision to Health Care Support Workers.
* To act as a key contact point for all staff regarding operational and administrative issues regarding the Health Care Support Workers.
* To be flexible in working patterns to provide support to cover shift patterns, weekend working and public holidays.
* To carry out care/clinical duties under indirect supervision of a Registered Professional. Acts on own initiative in delivering patient care in the Community. Supervision is available as required and work is within well established procedures and the Trust’s clinical supervision policy.

Essential person specification requirements

* GCSE English and Maths or equivalent
* NVQ level III in health and social care or equivalent experience
* Certificate of higher education (equivalent to 100 CAT points)
* Experience of working as part of a team
* Experience coordinating carer visits
* Experience working as a carer / in a caring environment
* Dealing with the public
* Using initiative
* Experience of delegating duties
* Supervision / appraisal of staff
* Confidentiality
* Computer Literacy
* Understand the need to establish & maintain a safe working environment
* Evidence of the use of reflective practice for personal development
* Awareness of how communication can be adapted to respond to a variety of situations
* Able to communicate clear and concise information both verbally and in writing.
* Ability to analyse and evaluate information.
* Ability to develop monitor & evaluate Health Care Support Workers input.
* Ability to prioritise workloads.
* Able to manage staffing levels to cover care service needs.
* Ability to manage service capacity for care patients.
* Ability to work on own initiative and as part of a team.
* Ability to work under pressure.
* Ability to demonstrate a flexible approach to working practice.
* Ability to adapt to new environments and changing situations.
* Ability to use negotiation skills and assertive skills appropriately.
* Ability to manage time effectively.
* Experience of IT systems.
* Willingness to undergo any relevant training.
* Full Clean Driving Licence and access to a car for business use in conjunction with work.
