* Develop support plans with service users and review them regularly to reflect changing needs.
* Encourage independence, enabling people to make informed decisions about their lives.
* Collaborate with external agencies and partners to ensure holistic, joined-up support.
* Maintain accurate records and ensure risk assessments, plans and reviews are kept up to date.
* Contribute to creating a safe, supportive, and homely living environment.
About You
We are looking for someone who is compassionate, resilient, and motivated to help people overcome barriers and achieve their goals.

* Experience of working with people experiencing emotional distress and/or mental health challenges.
* Strong communication skills and the ability to build trusting relationships.
* An understanding of safeguarding for adults.
* A reflective, organised and resilient approach to your work.
Desirable but not essential:
* Experience in supported housing.
* Knowledge of welfare benefits.
* Ability to communicate in Welsh.
* Good knowledge of the Ebbw Vale and Blaina areas.
What We Offer
* Comprehensive induction and ongoing training to support your development.
* Opportunities to progress and build a career in supported housing and social care.
