Job Summary

This post will be set in the rapidly expanding respiratory department of a large teaching hospital. The fellows will be an integral part of the lung cancer team supported by five consultants, two interventional fellows (THIS POST), patient navigator, specialty nurses and administrative staff. The role supports the National Optimal Lung Cancer Pathway (NOLCP) with access to same-day radiographs and CT scans, followed by same-day clinic and diagnostic sampling. There is a high volume of outpatient referrals and over 400 cases of lung cancer diagnosed each year. Fellows are expected to attend NOLCP clinics, contribute to the multidisciplinary meetings, and participate in bronchoscopy lists and inpatient referrals (supervised according to competence). The aim is approximately 2.5 clinics per week along with a defined number of bronchoscopy lists and inpatient referrals.


Main duties of the job

* RCR level 1 thoracic ultrasound certification required.
* Teach and assist in pleural procedures to provide opportunities for foundation doctors, core trainees and pre-RCR level 1 respiratory trainees.
* Complete relevant BTS national audits and participate in multi-centred national pleural disease trials.
* Develop a research portfolio through observational and retrospective studies.
* Competence in standard flexible bronchoscopy.
* Teach Plymouth University Medical students with organisational duties, small group teaching, bedside teaching and assessments.
* Propose a special study unit for medical students in a relevant area (lung cancer or pleural disease).
* Encourage enrolment for a postgraduate diploma in medical education with time allocated to complete this.
* General medical experience with a normal working week; potential support for out-of-hours rota at the candidate’s discretion (excluding nights).
* Non-UK graduates without MRCP must provide evidence of appropriate knowledge, training and experience.

Job Description


Job responsibilities – Pleural work

The pleural service sees over 1,000 referrals per year. Candidates must have RCR level 1 thoracic ultrasound certification, and during the attachment a logbook will be created to target requirements for RCR level 2. This is expected to be achieved through pleural clinic lists including mesothelioma clinics, consultant-supervised lists, and enhancement of pleural procedural skills (including guided procedures, pleural biopsies, lung biopsies, neck ultrasound and nodal biopsy, and indwelling catheter management). A medical thoracoscopy started in 2019.

* Fellows will teach and assist in pleural procedures and contribute to audits, trials and research.


Endobronchial ultrasound

Nearly 300 procedures are performed annually. Training will be gained by joining the lung cancer team, with patient selection, clinic involvement, radiological interpretation and consultant-supervised lists. Competence in standard flexible bronchoscopy is required.


Other elements

Involvement in teaching Plymouth University Medical students, including organisational duties, small group teaching, bedside teaching, assessments and examinations. A special study unit proposal for medical students in a relevant area is welcome. Consideration of a postgraduate diploma in medical education with time allocated to study.


General medical experience

Expect a normal working week with daily general medical experience on the wards. Opportunities to support the medical registrar out of hours rota (excluding nights) at the candidate’s discretion.


Essential requirements

Specialty trainees in Respiratory medicine or equivalent, nearing competency in bronchoscopy and holding RCR level 1 Thoracic ultrasound. Non-UK graduates without MRCP must provide written evidence of knowledge, training and experience.


Person Specification


Essential

* Specialty trainee in Respiratory Medicine
* Membership of the Royal College of Physicians
* Current GMC registration
* Good acute and general medical knowledge; ability to manage acutely unwell patients
* Advanced life support (ALS) certification
* Competent in outpatient clinic settings
* Experience with pleural/bronchoscopic procedures (Level 1 ultrasound)


Desirable

* Full driving licence or mobility
* Experience in lung cancer and pleural clinics; MDT involvement
* Postgraduate Diploma or higher in teaching (planning or obtained)
* Holds GCP certification; prior research engagement
* MD or PhD in relevant field
