For part-time roles, the salary and payments will be pro rata based on contracted hours. The post-holder will be a specialist social worker, allocated to the Adult Social Care Hospital Discharge Team (Hull Royal Infirmary and Castle Hill Hospital), with a focus on enhancing current provision and increasing safe and timely hospital discharges. This role will primarily support patients with alcohol and/or substance misuse issues who require care and more intensive support upon discharge. The social worker will: Support vulnerable adults with alcohol and/or substance misuse needs in accessing appropriate community services (e.g. mental health, substance misuse, housing) following hospital discharge. Facilitate safe and effective discharge planning, ensuring individuals are connected with the right support to meet their health and social care needs. Work collaboratively with hospital teams and community partners to reduce the risk of readmission and promote long-term recovery and stability.
