Full Time Hours: 37.5 per week Full Time Salary Range : 37,184.90 - 44,471.72 (based on full time hours, pro rata for part time) * Please note: Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours. The post-holder will be a specialist social worker, allocated to the Adult Social Care Hospital Discharge Team (Hull Royal Infirmary and Castle Hill Hospital), with a focus on enhancing current provision and increasing safe and timely hospital discharges. This role will primarily support patients with alcohol and/or substance misuse issues who require care and more intensive support upon discharge. The social worker will: Support vulnerable adults with alcohol and/or substance misuse needs in accessing appropriate community services (e.g. mental health, substance misuse, housing) following hospital discharge. Facilitate safe and effective discharge planning, ensuring individuals are connected with the right support to meet their health and social care needs. Work collaboratively with hospital teams and community partners to reduce the risk of readmission and promote long-term recovery and stability. You’ll be a vital part of our mission to improve health, wellbeing, and reablement outcomes for people across Hull. No two days are the same at Change Grow Live.
