All-Age Complex and Continuing Care are a team of clinical, business, commissioning specialists who provide continuing healthcare (CHC) children and young people's continuing care (CYPCC) and complex care commissioning to individuals across the Thames Valley footprint.

Our operating model has three main areas of activity:

Neighbourhood teams work alongside health and social care partners within the community assessing, reviewing and case managing care for adults and children within Oxford, Buckinghamshire or Berkshire, working directly with individuals and families.

Central services teams act as the front door and end-to-end service delivery, managing referrals into the service, business, administration and invoicing functions, working closely with individuals and their families.

Commissioning, contracting and brokerage team work closely with care providers and social care to commission individualised care for individuals.


Main duties of the job

This post has subject matter expertise and specialist knowledge to assess children and young people in the community/home and hospitals, utilising the national assessment tools and adhering to the required timescales. The role oversees, coordinates, participates and supports the assessment process with the multidisciplinary team (MDT). It links closely with the CYPCC team and determines individuals' eligibility in line with the National Framework for Children and Young People's Continuing Care (2016).

The role requires occasional travel to carry out assessments and reviews. Access to transport is therefore necessary, be it personal or public.


About us

It is proposed that this post will be in scope to transfer to Frimley Health NHS Foundation Trust (FHFT) on 1 September 2026. If appointed prior to that date, you will join Thames Valley ICB initially, then your employment will transfer to FHFT in accordance with TUPE/CoSOP regulations. If your start date is on or after 1 September 2026, you will be employed directly by FHFT from commencement.


Job responsibilities

* Support the commissioning of health‑care packages at home for CYP shared‑care eligible children and other community settings to prevent hospital admissions; help support early discharge from hospital wherever possible, working with clinical practitioners.
* Act in a deputising role in the absence of a senior CYP clinical manager, undertaking duty management, responding to package breakdowns, enquiries for new referrals, checklists and prioritising workload.
* Help support the daily duty system for CYP shared‑care, working with duty nurses and responding in a timely manner to support the caseload.
* Act as a member of the multidisciplinary team to coordinate the process, establish eligibility and assist in identifying a suitable care package or respite setting.
* Ensure that the review process for all commissioned care is undertaken in accordance with current guidance.
* Assist with monitoring and providing assurance on the quality of service providers for Children and Young Peoples Continuing Care, including residential placements and short‑break environments.
* Advise AACC Procurement on clinical needs and best practice in relation to services for children with complex health needs and provide clinical input to all parts of the commissioning process.
* Assist with the personal health budget (PHB) process for children’s families/carers who would like to receive a PHB and ensure that NHS Surrey & NHS Sussex procedures are monitored.
* Plan, manage, coordinate and undertake annual reviews of children and young people eligible for Continuing Care, responding to reported health needs and reviewing the DST and care package accordingly.
* Communicate complex information with empathy and consideration to children, young people, parents and carers, including diagnoses, prognoses and funding issues; handle end‑of‑life discussions sensitively.
* Work alongside local hospices, social services and specialist educational schools to provide joint care packages where appropriate, ensuring equitable service for the child.
* Participate in ongoing professional development and clinical supervision, including safeguarding supervision.
* Assist CYPCC team in delivering informal educational sessions to varying staff groups on subjects related to children’s continuing care.
* Organise own workload under oversight from a Clinical Manager.
* Follow local safeguarding policy and child‑protection procedures, provide reports and attend conferences as necessary.
* Support transition from CYPCC to the Adults CHC pathway.


Project Management

* Contribute to performance improvement, taking a lead on identified areas as agreed.
* Provide coordination of and participate in relevant internal and external working groups and provide project advice, expertise and support.
* Offer specialist advice and guidance on own portfolio of projects/functions.
* Investigate causes of variance from target/plan and proactively contribute to implementation of solutions.
* Manage risk and issue tracking mechanism and its proactive resolution and escalation processes.


Financial and Physical Resources

* Provide regular reporting on targeting of resources and monitoring their implementation from a value‑for‑money perspective.
* Oversee and monitor all aspects of team budgets.
* Evaluate projects/functions within identified portfolio for delivery against financial recovery/savings plans through sophisticated, high‑quality analysis.
* Take the lead in coordinating training, development and recruitment activity across the team.
* Manage colleagues, including appraisals, recruitment and induction, and progress informal/formal processes as necessary.


Information Management

Devise and provide improvements to current management information, analysing, reporting and suggesting procedures to enhance decision‑making processes, ensuring timely and accurate information analysis and reporting to management on agreed areas of work.


Research and Development

* Contribute to development of key performance indicators for the successful assessment of individuals and workstream success.
* Delegate aspects of research and development activities, collate information, analyse and report findings.


Planning and Organisation

* Contribute to strategic planning of team projects, identifying interdependencies across projects/functions, potential impacts on wider organisation, resource requirements and building contingency and adjustments.
* Contribute to development of performance and governance strategies and implementation of improvement programmes, in accordance with ICB and regional priorities.
* Contribute to short, medium and long‑term business plans, achieving quality outcomes.


Policy and Service Development

* Review and develop existing information management systems and contribute to an integrated approach to service or initiative management.
* Develop policies and procedures in own work function with an impact on the wider organisation as required.

Person Specification


Qualifications

* Educated to degree level in a relevant subject or equivalent qualification, with up‑to‑date registration with the relevant regulatory body (e.g. NMC Registered Nurse or Children's Nurse or allied professional such as Registered Physiotherapist, Occupational Therapist or Speech and Language Therapist).


Knowledge and Experience

* Significant experience of successfully operating in a politically sensitive environment.
* Previous experience of working in a health or care environment (including acute, community, mental health, local authority etc.) and evidence of continuing professional development.
* Experience and understanding of health and social care environment and roles and responsibilities within it.
* Experience of working in multidisciplinary teams.
* Comprehensive knowledge of current national and local policy/Framework Guidance relating to NHS National Framework for Children and Young Peoples Continuing Care and its application in practice.
* Knowledge and experience of working with safeguarding issues and policy guidance.
* Ability to address complex or sensitive issues in an organised and appropriate manner, both verbally and in writing at all levels; produce written documents and spreadsheets.
* Demonstrated ability to manage own workload effectively and make informed decisions in the absence of required information, working to tight and often changing time‑scales.


Skills, Capabilities and Attributes

* Ability to prepare and produce concise yet insightful communications for dissemination to senior stakeholders and a broad range of stakeholders.
* Numerate and able to understand complex financial issues combined with deep analytical skills.
* Knowledge of financial systems (e.g. monitoring budget management, processing invoices and procurement).
* Ability to act upon incomplete information, using experience to make inferences and decisions.
* Ability to analyse very complex issues where material is conflicting and drawn from multiple sources.
* Demonstrated capability to plan over short, medium and long‑term time‑frames and adjust plans and resource requirements accordingly.
* Demonstrated capability to manage own workload and make informed decisions in the absence of required information, working to tight and often changing time‑scales.
