Whether Full Time or Part Time, your role will be key to supporting individuals with mental health needs as they transition from children’s to adult services. You will play a vital part in empowering young people, working closely with families and a multidisciplinary team. Your work will focus on ensuring a smooth and person-centred transition, promoting independence and holistic well-being. The role will involve liaising with CAMHS, children’s services, and adhering to Care Act principles to enhance the independent living skills of young individuals.
