Overview
As a Social Care Practitioner (Social Work), you will provide support and promote independence for vulnerable adults in our community, assisting them in their time of need.
What You Will Be Doing
* Assist in organizing and providing effective care services, including assessment and review of care plans.
* Work under the direction of Social Workers to contribute to and review plans and services to improve intervention ideas.
* Protect vulnerable people by referring issues in accordance with safeguarding procedures; maintain confidential reports to professional and council guidelines.
* Communicate effectively with service users and providers, providing advice and guidance when necessary.
* Collaborate daily with Social Workers and external contacts, such as education services and health agencies, to ensure strong working relationships.
What We Need From You
* Experience in a relevant area of statutory or voluntary sector work in health and social care.
* Knowledge of relevant legislation, regulations and codes of practice; processes and procedures; support mechanisms and specialist equipment.
* Ability to gather information from a variety of sources and follow the assessment process.
* Strong communication skills with service users and other professionals to promote independence and personal choice, while understanding service users’ needs.
* Ideally, a relevant qualification e.g. NVQ Level 3 in a health or social care field.
Advanced Social Care Practitioner
* Minimum of 2 years experience in a relevant area of statutory or voluntary sector work in health or social care.
* Relevant qualification, e.g. NVQ Level 4 in health, social care or equivalent.
What You Need To Know
* To be considered for the Advanced Social Care Practitioner position, evidence must be clearly detailed in your application. Salary bands are dependent on experience:
* Social Care Practitioner: £29,540 - £32,061
* Advanced Social Care Practitioner: £32,597 - £35,412
* This role is hybrid, with a mix of office work at Badminton Road, Yate and occasional home-working.
* You will be expected to travel throughout the authority and surrounding area; you must have a full, valid driving licence with regular access to a vehicle or an alternative means of travel.
* Due to the nature of this role, an enhanced Disclosure and Barring Service (DBS) check is required for this post.
