About the Role
As a Band 5 Occupational Therapist, you will play a pivotal role in supporting the development of our Dementia and Mental Health Services to improve patient outcomes and experiences. You will be expected to assess the functional impact of both physical and mental health. Work in partnership with colleagues, as part of the multi‑disciplinary team to manage a caseload, using evidence‑based principles to assess, plan, implement and evaluate interventions in the community.

What We're Looking For
* A qualified Occupational Therapist registered with the HCPC.
* Interest or experience working with Older Adults, ideally with a focus on dementia care and/or Mental Health.
* Commitment to patient‑centred care and service development.
