About the role
As a Wellbeing Assessor, you will undertake assessments of patients in order to understand their individual needs and identify the most appropriate support for them, working collaboratively with other partners in the Primary Care Mental Health Service and associated organisations.
* Hours: 18.5 hours per week (one day must be a Thursday from 9.00am to 5.00pm)
* Location: GP Surgeries within Eastleigh Health PCN
About you
You will have experience of working with vulnerable people who have diverse need. You will have experience of working in a mental health setting.
You will need good communication, IT and teamwork skills.
