Opportunity for an OT/Social Worker or Nurse
Location: Havering Borough
An exciting opportunity has arisen for a passionate and dedicated Occupational Therapist (OT), Social Worker, or Nurse to join an innovative project within the borough of Havering. This project focuses on addressing the needs of individuals with a dual diagnosis who frequently present at crisis services, such as A&E and ICAH.
Key Responsibilities:
1. Develop Collaborative Care Plans: Work alongside service users, GPs, CGL, and mental health teams to create holistic, universal care plans aimed at improving long-term health and wellbeing.
2. Crisis Prevention: Identify gaps in community support to prevent the recurrence of crises experienced by individuals due to their dual diagnosis and associated challenges.
3. Collaborate Across Services: Partner with local GP practices within the North PCN, NELFT services, drug and alcohol services, and other support agencies to create tailored care pathways for individuals.
4. Data Analysis & Feedback: Analyse data from crisis contacts to identify patterns and recurring issues, and provide feedback to senior leadership and Quality Improvement teams for service enhancement.

Main duties of the job
Key Responsibilities:
5. To support the Havering North PCN identify gaps within the pathway for clients who have a dual diagnosis. The link with the GP and MHWT for those with dual diagnosis. To review the data to identify frequent attenders who have a dual diagnosis. To support with completing the universal care plan with the appropriate team and client. To establish more robust links with GP, CGL and MHWT to support the client for an agreed care pathway. Review patient pathways from referral to treatment, highlighting gaps or missed opportunities as well as any good practice. Psychiatric liaison in establishing pathways for those with dual diagnosis and MH needs who may need that extra support. Review existing care plan and update with UCP ensuring the voice of the client, carer and patient network are incorporated. To attend joint clinics with GPs and CGL with a view to establishing clinical pathway for care across all organisations. Identify additional community resources that can be supportive to both clients, carers and professionals. To review, assess and manage risk where needed and ensure that immediate support needed. To support with directing referrals to MHWT and ensuring that effective interventions are undertaken and communicated with all involved in the care. To work with the QI team in presenting the established data to the leadership team at the end of the project. To share learning with the other London boroughs.
