Clinical Nurse Specialist in Heart failure working within patients who present with heart failure both in the out-patient and in-patient service at the Princess of Wales Hospital Bridgend.
This post is fixed term for 6 months due to funding

An excellent opportunity has arisen to join our well established, motivated and forward thinking respiratory medical/ heart failure team at the Princess of wales Hospital in Bridgend as an Advanced Clinical Practitioner.
Clinical:
The CNS will be expected to provide support the consultant in the assessment, diagnostic investigations and management of patients referred to the Heart failure service as outpatient or inpatient. They will need be able to undertake a clinical examination, request and interpret biochemistry, haematology, histology and microbiology and echocardiogram results.
Education- provision of education to a multi-professional audience around specialist area of practice and to patient and families. Maintenance of own education and a live portfolio of evidence in line with HEIW Enhanced and Advanced Framework for Wales and HB requirements. The candidate is expected to have completed or be willing to work toward Level 7/Masters level clinical education and non-medical independent prescribing qualification.
Leadership- influence and support service change. Influence the agenda around specialist area of practice locally and nationally. Support the development of clinical guidelines and policy relevant to heart failure.
Research- use of Evidence Based Practice. Undertake/ support
Person Specification
Education
* In receipt of or willing to work toward Level 7 Education to support clinical role
* Can demonstrate ability to develop teaching packages and deliver teaching to a multi-professional audience and patients/relatives Maintains a live portfolio of evidence supporting the 4 pillars of Enhanced/Advanced practice
* Holds a valid education/teaching qualification Qualification in the assessment and management of heart failure
* Welsh Language Skills Speaking/Listening at Level 3
Clinical
* Evidence of working in cardiology/heart failure with heart
* Non medical prescriber
Research
* Demonstrates ability to undertake a Qip/Audit based project
* Demonstrates ability to use evidence based practice to support clinical management
* Demonstrates an understanding of how to undertake a Qip/Audit based project
Leadership
* Demonstrates ability to support change in practice
* Demonstrates ability to work autonomously in clinical practice
* Demonstrates ability to work within the context of a multi-disciplinary team
* Holds a leadership qualification
