REABLEMENT CARE WORKER – JOIN OUR TEAM IN CO. DURHAM
Salary: £13.30 Location: Across County Durham
Shifts: 7am–3pm / 3pm–11pm
Work Pattern:
* Week 1: Monday–Sunday (with two weekdays off)
* Week 2: Monday–Friday Guaranteed Hours Role

What You'll Do
As a Reablement Care Worker (also suitable for those with experience as a Care Assistant, Support Worker, Home Care Worker, HCA) you will:
* Visit individuals in their homes to support safe, independent living
* Assist with personal care, daily living tasks, and light household duties
* Support with mobility, routines, and reablement goals
* Accompany individuals to local amenities or appointments
* Work closely with the Hospital to Home team to support smooth discharges
* Follow personalised care plans and record progress
