Social Worker - Integrated Rapid Response Team
Full-time Social Work post within Bexley Integrated Rapid Response Team. The role is with the London Borough of Bexley as part of Adult Social Care.
Overview
The BRRT (Bexley Rapid Response Team) is a multi-disciplinary team comprising Nurse, Occupational Therapists, Physiotherapists, Doctor, Social Workers, Social Care Assistants, Therapy Assistants and administrative staff. BRRT operates seven days a week from 9:00–17:00 and 10:00–18:00. The team provides rapid support to Bexley residents in the community, undertakes holistic joint assessments of health and social care needs, reduces unnecessary hospital or residential admissions, and offers crisis intervention to ensure safety and wellbeing.
About the role
We require Social Workers who can work effectively within a multi-disciplinary team and autonomously when required. The role requires the ability to work at a fast pace with resilience to cope with the demands of a busy team. You will assess the needs of residents (and their carers) to establish creative, practical care plans that promote choice, control and independence.
Responsibilities
* Provide rapid support to residents in the community and undertake holistic joint assessments of health and social care needs.
* Help reduce admissions to hospital or residential care and provide crisis intervention to ensure safety and wellbeing.
* Collaborate with a multi-disciplinary team and work autonomously when required.
* Assess the needs of residents and carers and establish creative, practical care plans that promote choice, control and independence.
Qualifications and requirements
* Ability to work at a fast pace in a busy team environment.
* Experience and resilience to cope with demands of a Rapid Response environment.
* Positive, forward-thinking attitude with the ability to work collaboratively as part of a team.
* Appointment to this post will be subject to Enhanced Disclosure via the Disclosure and Barring Service (DBS).
* This post is subject to the English language requirement for public sector workers; appropriate standards are set out in the Job Description and Person Specification and will be applied during recruitment and probationary stages.
* Evidence of eligibility to work in the UK as required by the Local Authority and safeguarding commitments for children and vulnerable adults.
