Job Summary
Due to the partial retirement of a long-standing member of staff we have an exciting opportunity for a new colleague to join our team of Paramedic Practitioners. The successful candidate will be responsible for the provision of paramedical services, working as part of the practice's multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The paramedic/paramedic practitioner will be required to work autonomously in the practice and in the community (home visits), providing a range of services such as assessment, diagnosis, treatment, telephone triage and referring patients appropriately, as well as supporting the management team in the reviewing of clinical policy and procedure.
We are a SystmOne practice and utilise AccuRx and eConsult technology.
Responsibilities
* Triage patients and provide the necessary treatment during appointments in the practice and at home visits
* Examine, assess and diagnose patients and provide clinical care/management as required
* Identify, signpost or refer patients at risk of developing long-term conditions, preventing adverse effects on the patient's health
* Provide routine care to patients as required in accordance with clinically based evidence, NICE and the NSF
* Ensure continuity of care, arranging follow-up consultations or reviews as necessary
* Support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice)
* Liaise with internal teams and external services/agencies to ensure that the patient is supported appropriately (vulnerable patients, safeguarding matters, etc.)
* Ensure that read codes are used effectively and that clinical records are maintained in line with extant legislation
* Deliver opportunistic health promotion where appropriate
* Collect pathology specimens as required
* Chaperone patients where necessary
* Act as the lead clinician when dealing with clinical emergencies and recognise, assess and refer patients presenting with mental health needs
* Understand and adhere to relevant patient group directives and local clinical pathways
* Support the clinical team with safeguarding matters, substance abuse policies and related local/national policies

Primary Responsibilities
The paramedic practitioner will work within their scope of clinical practice. Responsibilities include but are not limited to the following:
* Identify, signpost or refer patients at risk of developing long-term conditions
* Provide routine care to patients in accordance with evidence, NICE and the NSF
* Examine, assess and diagnose patients and provide clinical care/management as required
* Triage patients and provide treatment during home visits and at minor illness clinics
* Collect pathology specimens; maintain accurate clinical records; ensure read codes are used effectively
* Ensure continuity of care and follow-up reviews
* Review treatment effectiveness and make changes to improve patient outcomes
* Chaperone as necessary; prioritise health issues; intervene appropriately
* Lead clinician in clinical emergencies; recognise mental health needs; liaise with external services
* Adhere to patient group directives and local clinical pathways
* Deliver opportunistic health promotion
Secondary Responsibilities
In addition to primary responsibilities, may be requested to:
* Support the practice audit programme
* Support junior team members and participate in local initiatives
* Contribute to shared learning and respond to national policies
* Participate in root cause analysis for significant/near-miss events
* Maintain currency and personal development plans
Person Specification
Experience
Essential: experience in triage; experience with a range of clinical conditions; experience in a primary care environment. Desirable: experience as a Paramedic Practitioner.
Qualifications
Essential: Registered Paramedic (HCPC) or registered with NMC. Desirable: Postgraduate diploma or degree (primary care).
Clinical Knowledge and Skills
* DBS check; Occupational Health clearance; HCPC registration
* Ability to triage, assess and diagnose; wound care; ECGs
* Chaperone procedures; pathology tests and results interpretation
* Diabetes, Hypertension, Asthma, CHD; accurate clinical notes
* Ability to work within own scope and know when to refer; PGDs
* Excellent communication, telephone manner, IT skills (Office/Outlook)
* SystmOne experience; time management; teamwork and autonomy
* Problem-solving and analytical skills; ability to follow clinical policy
Desirable
* Immunisations (routine, childhood and travel); Spirometry
* Knowledge of public health in the local area; health-promotion strategies
* Strong IT skills
