Overview

Change Grow Live are a charity dedicated to the belief that we can make a difference to our Service Users lives, offering support and respect in a safe environment, treating each user as an individual and working with them to find the right treatment and care options. We are advertising the role of Young Adults Resilience Worker to support the ongoing development of pathways for 18–25-year-olds.

Working with young adults aged 18 to 25, you will lead the delivery of high-quality provision across this distinct and diverse age group, with a focus on access, engagement, and support via a range of 11 and group interventions, inclusive of assessment & care planning. The role seeks to empower those aged 18–25 to be more resilient and risk aware, in turn supporting the likelihood of a safer, healthier and happier life.

Where: We have three main bases across Nottinghamshire, and you’ll work across the whole county via an outreach-based model of support. Hours: Full time, 37.5 hrs per week. Contract Type: Permanent.


About the Role

* Engaging young adults in a pragmatic and meaningful way, empowering them to access ongoing support.
* Work holistically to assess needs and develop a collaborative package of support for young adults, including brief, harm reduction, 11, group, and clinical interventions.
* Carry out comprehensive service user assessments and develop individual care plans that clearly identify how achieving each goal will enable progression in addressing substance use and related outcomes.
* Work closely with your service’s Safeguarding Lead to ensure all issues and concerns are escalated and dealt with quickly and effectively.
* Carry out risk assessments and risk management plans.
* Be responsive to the needs of young adults, ensuring interventions are accessible (e.g., facilitating home visits, working with colleges, universities, etc.).


About You

* Experience of working with young people and/or young adults
* Knowledge of issues facing young people and young adults today
* Knowledge of safeguarding concerns in relation to children, young people and adults
* Understanding of drug and alcohol issues
* Experience of forging strong working relationships with internal and external partners
* Access to a car and full driving license
* Knowledge and experience of working with young people/young adults who use substances
* Experience of delivering training to different audiences in a variety of settings
