What the Job Involves Your role involves managing a service caseload that includes customers with complex needs and ensuring compliance with the professional code of conduct under relevant legislation. You will accept referrals, conduct assessments and reassessments, and apply eligibility criteria as identified in the Care Act. The position requires you to build effective relationships with both internal teams and external partners, uphold the highest standards of quality in all your duties, and contribute to the knowledge-sharing and professional development within your team. You will act as a point of contact and resource for both the community and your colleagues, promoting safety and wellbeing through informed decision-making.
