Contract: Fixed one year contract (1st July 2026 – 30th June 2027)

At Encompass Southwest, we believe people should not have to fight to get the support they need.
Since 2021, we have worked alongside Royal Devon University Healthcare NHS Foundation Trust to deliver Neighbourhood Flow — a person-centred, trauma-informed service that helps people facing complex challenges access practical support, improve wellbeing, and avoid unnecessary hospital admissions.
This is not traditional social prescribing.
Neighbourhood Flow offers dedicated 1:1 support over a six-week period, building trusted relationships with people and helping support “flow” around them. We focus on what matters to the person — not just what services are available.
We are now looking for compassionate, proactive and relationship-focused Key Workers to join our growing team across Devon.
About the Role
As a Neighbourhood Flow Key Worker, you will support people who may be experiencing a range of challenges affecting their health and wellbeing, including isolation, housing issues, poverty, mental health difficulties, or difficulties navigating services.
You will:
1. Build trusted relationships with patients
2. Hold “What Matters to Me” conversations
3. Help people identify goals, strengths and priorities
4. Connect people with local services, community support and opportunities
5. Work closely with NHS teams and VCSE partners
6. Help reduce avoidable hospital admissions and improve wellbeing outcomes
This is a highly rewarding role for someone who is passionate about person-centred support, trauma-informed practice, and making systems work better for people.
What You’ll Be Doing
7. Managing a caseload of patient referrals
8. Providing person-centred, solution-focused support
9. Coordinating support around the individual
10. Working collaboratively with community nursing teams and health professionals
11. Connecting people with local organisations and resources
12. Recording case notes and outcomes accurately
13. Facilitating “Team Around the Person” meetings where appropriate
14. Contributing to service development, evaluation and learning
Support is mainly provided by telephone, alongside home visits, community meetings, and attendance at NHS sites where needed.
About You
We are looking for someone who:
15. Has experience supporting people with multiple or complex needs
16. Can build strong relationships with people from all backgrounds
17. Understands trauma-informed and person-centred approaches
18. Is organised, proactive and able to manage a varied caseload
19. Has experience working with health, social care, community or voluntary sector services
20. Communicates confidently with professionals and patients alike
21. Is compassionate, adaptable and solution-focused
22. Is confident using IT systems and maintaining accurate records
You will also need:
23. A full driving licence
24. Access to a vehicle
25. Willingness to travel across Devon
