Making a Difference as Social Worker in Adults Services

Our Social Workers are pivotal to our teams. When you join us in Adult Services, you will undertake needs-led assessments and provide specialist support to adults, their carers, service users, and colleagues.

It is essential that you have knowledge and understanding of relevant legislation and guidance, risk factors, and difficulties relevant to client groups, as well as an understanding of rehabilitation processes, particularly for older people. Knowledge of user and carer issues and a thorough understanding of the care management process are also required.

To apply, you will need experience in developing care packages, working with those aged 18 and above, older/disabled people, and in a multi-agency setting.

Qualifications such as CQSW, DipSW, CSS, or BA Hons in Social Work are essential, along with a willingness to undertake all essential training.

This is an exciting opportunity to utilize existing skills and develop new ones. You will receive continuous support, development, and career progression opportunities. The posts will require you to regularly attend the St Helens office and travel around St Helens and surrounding areas.

Assessment and Review Worker Role

Contact Cares is a large, busy multidisciplinary team and the single point of access for health and social care referrals. We are recruiting a motivated Assessment and Review Worker to join our assessment and discharge team within the service. The team includes both qualified and unqualified Social Care, Nursing, and Therapy staff. We provide planned and urgent assessments, a 2-hour response under Urgent Community Response Criteria, and support hospital and intermediate care discharges.

The Contact Cares team also offers initial contact, care planning, onward referrals, and signposting services for a wide range of adult clients. Our focus spans prevention, early intervention, and reducing hospital admissions and readmissions, making the work varied and interesting.

This role is based within the hospital Discharge team at Nightingale House, working closely with health and social care professionals to facilitate safe and timely discharges from hospital and avoid admissions, with a focus on home first.
