We're looking for a passionate and skilled Transitions and Positive Behaviour Support (PBS) Facilitator to join our No Limits team in Nuneaton. This unique role combines person-centred transition planning with Positive Behaviour Support practice leadership, helping young people with learning disabilities, autism and complex needs thrive during their journey into adulthood.

About the Role Working across the No Limits provision, you will spend approximately three days each week supporting and developing Positive Behaviour Support practice and two days supporting transition planning and family engagement through our My Way programme.
You will build strong relationships with young people, families and professionals, ensuring every learner receives consistent, person-centred support that reflects their strengths, aspirations and individual needs.
As part of the No Limits team, you will work closely with colleagues, families and external partners to ensure young people experience positive transitions into, through and beyond our provision.
What You'll Be Doing Positive Behaviour Support Role model outstanding, person-centred support across the No Limits provision.
Work directly alongside learners requiring enhanced levels of support, helping them achieve positive outcomes and improved quality of life.
Complete behavioural assessments, functional assessments and PBS plans.
Support the collection, analysis and interpretation of behavioural data to inform practice.
Coach and mentor colleagues in real time, embedding PBS values and approaches into everyday support.
Develop the confidence and skills of teams through observation, feedback and reflective practice.
Work alongside the PBS Lead to maintain high standards of PBS across the service.
Promote proactive, preventative and strengths-based approaches that focus on understanding the person rather than managing behaviour.
My Way Transitions Act as a key point of contact for prospective learners and families during the pre-placement and transition process.
Build trusting relationships with families, supporting them through significant periods of change.
Coordinate person-centred transition planning for young people entering, progressing through and leaving No Limits.
Work collaboratively with local authorities, education providers, social care teams and health professionals.
Attend and contribute to EHCP reviews and multi-disciplinary meetings.
Support learners to identify and work towards meaningful aspirations for adulthood.
Help ensure every learner leaves No Limits with a clear and ambitious next step that reflects what matters most to them.
Promote independence, self-advocacy, community inclusion and citizenship throughout the learner journey.

About You We're looking for someone who combines warmth, curiosity and professionalism with a genuine commitment to person-centred practice.
You will bring:
Experience working within education, health or social care services supporting people with learning disabilities and/or autism.
Experience supporting individuals whose behaviour may challenge services.
Knowledge and understanding of Positive Behaviour Support principles and practice.
Understanding of transition planning and the challenges young people and families can face when moving into adulthood.
Experience contributing to or implementing Education, Health and Care Plans (EHCPs).
Excellent communication and relationship-building skills.
The ability to coach, influence and support others to develop their practice.
Strong organisational, planning and IT skills.
A creative, solution-focused approach to overcoming barriers.
A commitment to co-production, person-centred planning and working alongside people rather than doing things for them.
