Job Title: Social Worker – Learning DisabilityLocation: Hammersmith and Fulham
Pay Rate: £35 per hour
Job Description:
Liquid Personnel is seeking a dedicated and compassionate Adult Social Worker to join its clients Learning Disability Team at Hammersmith and Fulham. The successful candidate will provide support and services to adults with learning disabilities, helping them to achieve their full potential and improve their quality of life. This role involves assessment, planning and intervention to address the needs of individuals and their families.
What will your responsibilities be?
1. Conduct comprehensive assessments of clients’ needs and circumstances.
2. Develop and implement personalized care plans.
3. Provide counseling and support to clients and their families.
4. Collaborate with other professionals and agencies to coordinate care.
5. Advocate for clients’ rights and access to services.
6. Maintain accurate and up-to-date client records.

Qualifications & Experience:
To be successful in this role you must have,
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK driving licence
* Post-qualified experience
* Degree level or equivalent in Social Work
