Job Overview
We have an exciting opportunity for a Band 7 Diabetes Specialist Nurse to join our award‑winning proactive Hounslow Community Diabetes Service. The role involves improving care for people with type 2 diabetes (and type 1 in special circumstances) and delivering high‑quality, evidence‑based care through nurse‑led step‑up and step‑down clinics, virtual technology support, and joint care for house‑bound and patients requiring supported self‑management.
Responsibilities
* The post holder will work effectively as a member of the multidisciplinary team—including patients, specialist nurses, dietitians, consultants, primary care colleagues and community teams—to improve holistic health outcomes for patients with diabetes.
* Provide specialist care, advice and support through direct booking of primary care hubs (PCNs) and jointly support the development of integrated working.
* Provide joint practice‑based clinic sessions to optimise care planning and prescribing decisions for complex patients in GP practices.
* Triage and audit referrals to the service; provide relevant, evidence‑based feedback to referrers to enable timely care for patients.
Qualifications & Experience
Experience of working with virtual technology to support a flexible service in complex diabetes clinics and group settings is welcomed. Applicants should have:
* Demonstrated experience in diabetes specialist nursing, including step‑up and step‑down clinic care.
* Strong communication and leadership skills to work effectively in partnership with community and primary care colleagues.
* Ability to deliver respectful, evidence‑based care and facilitate self‑management skills for patients and carers.
Hours & Availability
The role is full‑time (37.5 hrs per week) but part‑time applications will also be considered in line with service needs.
