Employed by the Meon Health Practice (MHP), the post holder willbe a registered Paramedic with post-graduate qualifications relevant to theposition. Acting within their professional boundaries, the post holder willprovide care for patients presenting with undifferentiated conditions. Thiswill l include initial history taking, clinical assessment, diagnosis, treatment,and evaluation of care. They will demonstrate safe clinical decision-making andcare for patients within the MHP.

The post holder will demonstrate critical thinking in the clinicaldecision-making process.

They will work collaboratively within the Complex Care Team tomeet the needs of patients, supporting the delivery of policy and procedures.

They will work as an autonomous practitioner to provideclinical case management for frail patients with acute / chronic / palliativecare needs and at risk of further deterioration in health that may result in unplannedavoidable hospital admissions or unnecessary length of hospital stay.

This vacancy is working 22.5 hours over 3 days


Main duties of the job

Remote consulting and visiting MHP patients in CQC registered Carehomes (nursing / residential) and housebound patients registered with thenetwork practices to achieve acute visiting and assessment early in the day(typically before mid-day) to optimise patient outcomes.

The role will comprise twospecific disciplines:

1.Working within the MHP complex care team you will undertakeproactive consultations with Nursing / Residential home and housebound patientswithin MHP. You will provide advice to care home staff on patient care andwider health policy. This includes patient specific care plans and DNACPRdecisions.

2.Working alongside a centrally based duty GP you will deliver ahigh standard of patient care to patients who have been placed onto a centralGP triaged visiting list by practices within MHP

As a Primary Care ParamedicPractitionerwithin the complex care team, you will be using advancedautonomous clinical skills and a broad, in-depth theoretical knowledge base, toprovide patient care. This will include referring patients to hospital or othercommunity services. You will be working autonomously (in the patients home ora care home) with access to electronic patient records and available escalationto the complex care GP/ANP or duty GP for advice and support as required. Youwill work alongside Clinical Pharmacists from within MHP and the CCG to deliverthe appropriate medicines related care.

MHP delivers care to patients registered at either Whiteley, Jubilee or Highlands Practice in Fareham.

Job responsibilities

CLINICAL ROLE

Workalongside the complex care team to provide a first point of contact forpatients presenting with undifferentiated, undiagnosed problems, making useof skills in history taking, physical examination, problem-solving andclinical decision-making, to establish a diagnosis and management plan.

* Provide assessment, treatmentand diagnosis at point of first contact, by attending to patients in avariety of clinical or non-clinical settings according to patients needs.
* Undertake home/nursing andresidential care home visits, in accordance with the relevant protocols.
* Make professionally autonomousdecisions for which you will be accountable.
* Refer patients to an alternativecare setting or treat & discharge as appropriate.
* Instigatenecessary invasive/non-invasive diagnostic tests/investigations and interpretfindings/reports.
* Ensure that professionalstandards are maintained and within the guidance issued by the DoH(Department of Health), the HCPC (Health and Care Professions Council) andthe College of Paramedics (COP)
* Work withmulti-disciplinary teams within MHP and healthcare community, to promoteintegrated and seamless pathways of care.
* Contributeto MHP achieving its qualitative and quantitative targets to maintain highstandards of patient care and service delivery.
* Participate in the assessment ofcommunity health needs and help to develop patient and family-centredstrategies to address them.
* Help to develop and set up newpatient services and participate in initiatives to improve existing patientservices.
* Assist withdevelopment of patient care plans and ensure DNACPR are in place whereappropriate.
* Use Associated Clinical Groupsinformatics to identify patients needing additional care or at risk ofhospital admission.
* Assess frailty use Rockwoodfrailty scales/FI

B.Teaching and mentoring role: the post holder will desirably:

* Promote a learning environmentfor patients, colleagues and other health professionals.

C. Professional role

The post-holder will:

* Promote evidence-based practicethrough the use of the latest research-based guidelines and the developmentof practice-based research.
* Monitor the effectiveness oftheir own clinical practice through the quality assurance strategies such asthe use of audit and peer review.
* Maintain any professionalregistration such as with the HCPC,COP
* Participatein continuing professional development (CPD) opportunities to ensure that upto date evidence-based knowledge and competence in all aspects of the role ismaintained including maintaining up to date CPR training and other corecompetencies.
* Recordaccurate and timely consultation data within the patients records
* Keep up to date with pertinenthealth-related policy and work with the SHN team to consider the impact andpossible strategies for implementation.
* Work collaboratively withcolleagues within and external to MHP.
* Pro-actively promote theParamedic role within MHP and externally to key stakeholders and agencies.
* Identifyown learning needs in order to remain up to date and improve servicedelivery.

Non-clinical team role

The post-holder will:

* Identify appropriateopportunities to delegate both clinical and administrative tasks to otherappropriate staff
* Help MHP operate in acost-effective manner.
* Identify and manage care riskson a continuing basis.
* Participate in MHP meetings asrequired.
* Participate in audits andinspections as appropriate.
* Work closely with the doctorsand managers in the setting up and/or improving of MHP systems formonitoring/measuring performance against clinical governance and quality targets.
* Ensure all MHP policies arefully implemented.

E. Health & Safety and Risk Management

* The post-holder must comply atall times with MHP Health and Safety policies.
* The post-holder will comply withthe Data Protection Act (1998) and the Access to Health Records Act (1990)and GDPR.

F. Equality and Diversity

The post-holder must co-operatewith all policies and procedures designed to ensure equality of employment.Co-workers, patients and visitors must be treated equally irrespective ofgender, ethnic origin, age, disability, sexual orientation, religion etc.

G. Respect for patient confidentiality

The post-holder should respect patient confidentiality at alltimes and not divulge patient information unless sanctioned by therequirements of the role.

H. Communication and working relationships

The post-holder will establish and maintain effectivecommunication pathways with the following:

* GPs within MHP and externallywith the CCG as a whole
* Whole nursing team includingNurse Practitioners
* Reception and AdministrationTeams

I. Special Working Conditions

The post-holder is required to travel independently betweensites, patient homes and to attend meetings etc hosted by other agencies

Thepost-holder will have contact with bodily fluids, i.e. wound exudates, urine,blood etc whilst in clinical practice and must take precautions as requiredto minimise the risk of infection/contamination using PPE.
