Ortho- Geriatric Consultant

University Hospitals Birmingham (UHB) is seeking a Consultant Ortho-Geriatrician / Perioperative Geriatrician to join one of the largest and most ambitious trauma and perioperative services in the UK.

This post is part of a Board-supported expansion of frailty and perioperative medicine, reflecting our commitment to delivering outstanding care for older people undergoing surgery from fragility fractures to complex trauma.

We welcome applications from:

* Established ortho-geriatricians
* Geriatricians with a strong interest in perioperative medicine / POPS
* Consultants transitioning into ortho-geriatrics or trauma-focused roles

UHB manages approximately 1,000 neck of femur fractures annually, within the largest Fragility Fracture service in the UK and Europe. This scale is matched by depth of senior support and multidisciplinary expertise, including:

* A growing ortho-geriatric consultant team
* 38 Consultant Orthopaedic Surgeons
* Senior Fragility Fellowship
* Established advanced nurse practitioners and therapy teams
* Embedded Quality Improvement and governance infrastructure
* Strong links to major trauma, critical care and perioperative services at QEHB

Importantly, this is not a lone-working role. The service is designed to distribute clinical workload, protect consultant sustainability, and create capacity for leadership, education and development.


Main duties of the job

The post holder will be part of the multidisciplinary team approach to care with the specialty of Orthogeriatrician and will be supported by the resident doctor team within Orthopaedics. The post holder with support will continue to develop the role of geriatric medicine within the trauma unit, enhancing the patient experience, continuing with the quality improvements for patients as part of the Fractured Neck of Femur pathways.

This role offers a balanced blend of clinical care, leadership and development, tailored to the successful candidate's experience and interests.


Clinical Responsibilities

* Senior ortho-geriatric input to patients admitted with fractured neck of femur
* Review of orthopaedic outliers and complex trauma patients
* Multidisciplinary decision-making to optimise outcomes, reduce complications and length of stay
* Support for families and carers during high-risk decision-making


Perioperative Opportunities

* Contribute to perioperative pathways
* Align with national perioperative medicine priorities and models of care
* Explore links with major trauma and complex surgery


Job responsibilities

The appointee should have a special interest in care of the elderly with fragility fracture whether as in-patients or in the out-patient setting and also in Orthopaedic care for the elderly, specifically those admitted with Fractured Neck of Femur. The post will support collaboration between Surgeon and Geriatrician in the management of older people following trauma and enhance training for foundation doctors.

You will hold Full Registration with the General Medical Council.

You will work as part of a collaborative approach in providing the best quality of care for our patients within the Fractured Neck of Femur unit at Heartlands Hospital.

You will hold clinical responsibility for the patients in your care, ensuring that dignity, compassion and safeguarding are at the forefront of the patients care.

You will lead senior decision-making around patients care, supporting the learning of more resident doctors and trainees.

You will evidence and maintain a good knowledge and understanding of Governance.


Key Responsibilities

* Medical care of Orthogeriatric patients on our Trauma wards, predominantly fractured neck of femur patients.
* Maintaining the highest clinical standards in the management of Orthogeriatric patients on the trauma unit and other clinical areas, as required.
* To share with colleagues responsibility for the day-to-day management of the Orthogeriatric patients on the trauma unit, in line with national and local clinical guidelines.
* Teaching and training of resident staff, medical students and Allied Health Professionals.
* Having responsibility for ensuring active participation in continuing medical education (CME).
* Actively participating in both departmental and Trust matters concerning Clinical Governance and audit.
* Attending meetings as appropriate.
* Fulfilling duties of weekly timetables, including out of hours or weeks with prior agreement.
* Recording findings and generally investigating and preparing patients for Orthopaedic surgical operation or other treatment and maintaining the patients' medical care during their inpatient episode.
* Attending the daily Trauma meeting when on duty and may be required to present patients as needed.


Person Specification


Qualifications

* Hold MRCP or equivalent qualification. Evidence of continuing professional development. Degree Level education or equivalent. CCT in Geriatric Medicine and registered on the speciality register.
* Full registration with the General Medical Council.


Experience

* Experience in liaison geriatrics in the form of Ortho Geriatrics and surgical liaison.
* Broad experience in the management of all aspects of geriatric medicine.
* Demonstrates the underpinning subject specific competencies relevant to the role setting and scope.
* Clinically evaluates and manages a patient, formulating a prioritised differential diagnosis, initiating an appropriate management plan, and reviewing and adjusting this dependent on the outcomes of treatment.
* Good practical clinical skills.
* Good understanding of Clinical Governance.
* Good understanding of safeguarding. Practices with the professional values and behaviours expected of all doctors as set out in GMC Good Medical Practice and Generic Professional Capabilities Framework.
* Respects patients' dignity, ensures confidentiality and appropriate communication where potentially difficult barriers exist, including communication with parents/carers.
* Demonstrates key generic skills around the areas of consent, ensuring humane interventions, prescribing medicines safely and using medical devices safely.
* Adheres to professional requirements, participating in annual appraisal, job planning and reviews of performance and progression.
* Aware of legal responsibilities relevant to the role, such as around mental capacity and deprivation of liberty; data protection; Gillick competence: equality and diversity.
* Takes prompt action where there is an issue with the safety or quality of patient care, raises and escalates concerns, through clinical governance systems, where necessary, collaborates with multidisciplinary and interprofessional teams to manage risk and issues across organization and settings, with respect for and recognition of the roles of other health professionals.
* Evaluates and audits own and others' clinical practice and acts on the findings. Reflects on personal behaviour and practice, responding to learning opportunities.
