Offer description

Salisbury Hospital is expanding its Frailty Same Day Emergency Care (SDEC) service and we are looking for enthusiastic Advanced Clinical Practitioners (ACPs) to join our dedicated team.

This is an exciting opportunity for clinicians with a passion for caring for older people who thrive in fast-paced, dynamic environments. Working alongside Consultants, Therapists, and Community Partners, you will play a key role in delivering Comprehensive Geriatric Assessments within the Emergency Department and SDEC areas.

We are seeking motivated colleagues who value collaborative, patient-centred care and want to contribute to shaping and developing services for our older and frail population.

In this role we can offer you flexible working opportunities (We work a Mon–Fri service, with scope for varied start/finish times and part-time roles) in a supportive, forward-thinking team environment with strong multi-disciplinary collaboration.

Provide advanced clinical practice and deliver Comprehensive Geriatric Assessments for older patients in ED and SDEC.

Utilise your full ACP skill set to assess, examine, diagnose and manage patients with undifferentiated presentations.

Support same day discharge and admission avoidance, working closely with community partners including virtual wards.

Develop expertise in managing common conditions affecting older people, such as Heart Failure, Falls, and Parkinson's Disease.

Contribute to service development as SDEC expands across the Trust.

Supervise, support and teach ACP trainees and colleagues within the multi-disciplinary team.

Join us at Salisbury and help shape the future of frailty care – where your skills make a real difference every day.

We are an acute Trust, rated "Good" by the CQC, with a track record of high performance providing regional and super regional specialist services such as: Burns, Plastics & Reconstructive Surgery, Wessex Regional Genetics Laboratory, Wessex Rehabilitation, Spires Cleft Centre and the Duke of Cornwall Spinal Treatment Centre. We have about 470 beds and employ over 4000 staff.

We support flexible working and will consider requests taking into account the needs of the service. We think working here is pretty rewarding. You can access a range of NHS discounts, receive a minimum of 35 days paid holiday (pro rata), and we offer a generous pension scheme. On-site benefits include car parking (fee applies), leisure centre, day nursery and holiday play scheme.

Salisbury is an attractive place to live and work and really gives you the best of everything. With easy access to London and local airports, the city of Southampton is only a short train or car journey away or if you prefer the sea, Bournemouth is also accessible.
